Many patients may have had limited access to consistent healthcare prior to incarceration and may present with complex medical, behavioral health, substance use, trauma, or social needs. The role requires compassionate, trauma-informed care and the ability to manage both scheduled and urgent women’s health concerns within a secure correctional environment. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Vacation Cash-Out Option 3 Paid Personal Days 11 Paid Holidays Accrued Paid Sick Time Responsibilities Provide high-quality nursing care and medication administration to individuals in accordance with physicians' orders and agency protocols Serve as a clinical resource and mentor to other LPNs and direct care staff within assigned programs Support the onboarding, orientation, and training of nursing staff, including oversight of clinical competencies Monitor the implementation of health care plans and treatment protocols Assist in the scheduling and coordination of nursing coverage across programs, ensuring consistent and adequate staffing Communicate medical issues promptly and effectively to RNs, Health Care Directors, physicians, and program leadership Conduct or assist in health assessments and ensure timely documentation in Electronic Health Records (EHR) Accompany individuals to medical appointments when necessary, and ensure follow-up care is completed and documented Monitor and maintain compliance with Medication Administration Program (MAP) standards and provide MAP training as required Participate in incident reporting, follow-up, and resolution in collaboration with clinical and administrative leadership Ensure health records and documentation are complete, accurate, and compliant with DDS and agency standards Promote a culture of respect, safety, and dignity in all interactions with individuals and staff Perform other nursing or administrative duties as assigned by the supervising RN or Health Care Director Qualifications Education & Experience: Current and valid Licensed Practical Nurse (LPN) license in the state Minimum 2 years of nursing experience in a residential, community health, or developmental disability setting (leadership or supervisory experience preferred) Skills and Knowledge: Knowledge of MAP, DDS regulations, and EHR documentation practices Strong leadership, organizational, and communication skills Ability to mentor and guide other LPNs and support staff Proficient in basic computer skills and electronic documentation systems Valid driver’s license and access to reliable transportation Current certifications in First Aid , CPR , and MAP Commitment to person-centered care and promoting the rights and dignity of individuals served Why Join Seven Hills Community Services? Supervise the coordination of medical services and nursing staff in the Southeast. Institute appropriate follow up and communicate all medical issues to: Residential Manager, Health Care Director, Area Director, and any other appropriate Seven Hills Community Services (SHCS) staff. Maintain, Review, and supervise all medical care services in Southeast. Provide supervision and training to northeast nursing staff. Medication Administration Program (MAP) trainer and Medication Administration Program (MAP) oversight with reports to facilitate compliance. Attend and participate in residential staff meetings and provide in-service training of residential staff on medical topics as needed. Complete reports in accordance with regulations. Facilitate effective communication with participant’s physician to obtain medical information and treatment. Document medical needs of participants and interventions as needed. Advise the Health Care Director of any medical problems that may hinder a person’s progress or placement in current residential setting. Foster the growth of individuals and to treat them with respect, dignity and in an age and culturally appropriate manner. Be familiar with the regulations and statutes governing and insuring the human rights of program participants. Implement procedures and services including medical interventions, or additional duties, as assigned by the Health Care Director of Seven Hills Community Services (SHCS). Other duties as assigned. Qualifications Education & Experience: Registered Nurse with current MA license. 2-3 years working in a residential setting with disabled adults. Supervisory experience 2 years.
