As part of a support coordination team who will manage members enrolled in the Employment and Community First CHOICES (ECF CHOICES) program, the ECF CHOICES Support Coordinator will be the primary support coordinator for a panel of members with intellectual or developmental disabilities. Support coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. This is a home-based position with field responsibilities approximately 75% of the time.
