Manages the orientation and competency evaluation of nursing staff in collaboration with the Educational Nurse Coordinator. Connects individually with each employee, increasing employee engagement scores and satisfaction. Clinical Practice Serves as a role model, mentor, and expert resource to nursing and assistive staff in the provision of direct patient care. Provides leadership for the development, implementation, and evaluation of standards of nursing practice and interdisciplinary clinical guidelines. Facilitates and supports a nursing practice environment in which empowerment, responsibility, autonomy, accountability, collaboration, and peer review are exercised. Develops standard processes and protocols to minimize variation, support efficient workflows, and confirm the right work is performed by the right role. Support of Clinical Operations Participates in planning and problem-solving of daily unit operations and priorities of patient services. Attend all workload meetings, working with workload chairs, and discuss any findings and or action plans with the Sr. Director. Participates in the development, implementation, and evaluation of unit goals and objectives Manages, directly and in collaboration with others, staffing, scheduling, benchmarking, work redesign, and variances. Organizes and facilitates unit-based educational activities and in-services. Assures compliance with regulatory programs' standards related to nursing. Provides leadership, assists, and advises Administrators and Medical Directors, in satisfying other regulatory standards. Write, revise, interpret, and implement Michigan Medicine policies and procedures. Assists in the planning and implementation of new or expanded services and programs. Provides interface and liaison communications regarding programs external to the unit that have implications for nursing staff. Represents nursing, Ambulatory Care Services, and/or the clinical department(s) or unit(s) on assigned committees Quality Management and Research Participates in and conducts the analysis of data. Assists in the development of strategies to improve care management within the area or unit. Participates in and supports the involvement of nursing staff in unit quality improvement activities. Provides leadership to staff in understanding and applying quality improvement principles and the adoption of evidence-based changes in practice into daily work. Promotes the integration of research findings into clinical practice. Supports research activities in the unit, as indicated. Works closely with each team to define and measure productivity, holding teams accountable to expectations, and ensuring work is performed within their scope of practice. Professional Development and Education Facilitates the education of health sciences students. Develops self professionally through continuing education, professional reading, participation in professional organizations, and networking. Evaluates own performance periodically with the Senior Nursing Director. Required Qualifications* Required Educational Requirements Applicant must meet one of the following: Bachelor's of Science in Nursing OR Associate's degree or Diploma in Nursing and a Master's degree in Nursing Current licensure as a Registered Nurse in the State of Michigan Minimum five (5) years of RN experience Minimum two (2) years of recent (within the last 5 years) RN experience working with surgical patient population or neurosciences. Nurse Leadership certification or completion of a nursing leadership certification within two (2) years of accepting this position. Description Job Title - Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Responsibilities DESCRIPTION OF COMPETENCIES A. Operational Duties • Conducts in home and telehealth assessments for patients with complex needs, including formulating a diagnosis, ordering and interpreting diagnostics, and delivering appropriate care in line with patient preferences and goals of care • Prescribe new medications and perform medication reconciliation • Demonstrate sound clinical judgement and evidenced based practice in symptom management and end of life care • Facilitate family meetings, goals of care conversations, and ACP discussions • Leads oversight of care plans in conjunction with the rest of the clinical care team • Advocates for the patient and caregiver in collaboration with other care providers to keep care coordinated and consistent with patient goals • Serve as a link between health and social care services and community to improve access to palliative care and quality of care • Work closely with interdisciplinary team to address changes in patient needs or symptoms • Educates patient/family on trajectory of illness, use of medications, assesses disease understanding • Participate in weekly IDT meeting with care team • Excellent teamwork, communication and leadership skills • Documentation as required • Performs other related duties as assigned Qualifications Required: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. • Licensure: New York State License with current registration as a registered nurse. • State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: • a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. • b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). • c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR • Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours.
