Criteria A) Restraint Care 1. Initiates/evaluates alternatives to restraint prior to application 2.Applies restraints consistent with the approved procedure. 3. Monitors and assesses patient's response throughout the restraint period at the appropriate intervals. 4. Provides specified patient care (toileting, skin care, hydration, feeding, etc.) on a timely basis. 5. Provides consultation for peers to determine alternatives to restraints and 1:1 observation. 6. Documents restraint use and associated care thoroughly. B) Pain Management 1. Assess patient for presence of pain on admission and during assessments/reassessments. 2. Incorporates patient's cultural/spiritual beliefs regarding pain into pain management plan. 3. Implement pain management techniques. Focus on prevention rather than treatment. 4. Include patient and/or family members in developing a pain management plan. 5. Consider other methods of pain control when developing plan of care: massage, repositioning, immobilization, and music therapy. C) Abuse Assessment 1. Is aware of abuse recognition criteria and incorporates it into assessments. 2. Reports signs of possible abuse/neglect to the Administrative Director of Behavioral Health, physician, Risk Management and Social Work. 3. Takes appropriate action to support patient safety when signs of abuse are noted. 6) Demonstrates accountability for own professional practice. RN - Job Specific Duties: Complete the online or in person Adolescent/Adult approved SANE course within one month of hire - it is approx. a 40 hour course. Attend in person 4-8 hour class with SANE coordinator to review what was discussed during 40 hour course and discuss additional content that is specific to our SANE program @ Logan Health. Complete the Pediatric online or in person SANE course within two years of hire (approx. 42 hours) and/or when the SANE coordinator feels it is the right time and will benefit the SANE program. Participate in peer review process and present own Medical Forensic Exam cases at staff meetings –when requested by SANE Coordinator. Be available in person or by phone within 72 hours of doing a SANE – for peer review with SANE coordinator. Precept new nurses to SANE role when you are independently functioning as a SANE. Job Specific Duties: Develops, implements and coordinates individualized , goal directed nursing care to patients through the use of the nursing process as appropriate for RN’s as applicable to the assigned area(s). Maintains nursing standards of care and responds to changing workloads and patient conditions by: remaining calm, making objective assessments, prioritizing, and asking for assistance as appropriate . Transcribes and implements provider orders and treatments both verbal and written accurately and efficiently as noted by chart reviews and observation. Consistently problem solves issues affecting patient care. Exhibits a thorough practice of infection control procedures per policy and procedure as applicable to the assigned area(s). Identifies , verbally evaluates, and reports identified reaction changes in the stated physical and mental conditions of patients. Documents all encounters with patients seeking health advice according to organizational standards, while clearly demonstrating the nursing process. Provides individual instruction and education to patients and families concerning health and medical needs per department protocol and as applicable to assigned area(s). Assesses and anticipates the needs of the patient, and coordinates the necessary interventions as defined by policy, procedure and the written/verbal orders of the provider as applicable to assigned area(s).
