Job Description Schedule: This is a full-time, 5-day, 8-hour position (8:30 AM - 5:00 PM; CST/PST/MST preferred ), including one weekend shift per month. Candidates must be willing and able to consistently work within Pacific Standard Time (PST), Mountain Standard Time (MST), or Central Standard Time (CST) hours. A bit about this role: This position represents an amazing opportunity for an Advanced Practice Provider (NP/PA) to help build Devoted Medical’s Transitions of Care (TOC) Program. The TOC program provides virtual care in the 30 day window after a patient is discharged from the acute setting (hospital or skilled nursing facility). The clinical team is highly interdisciplinary and composed of coordinators, guides, nurse case managers, nurse practitioners, and a medical director. The program’s goal is to prevent readmissions by providing exceptional, comprehensive wrap-around care in the post-discharge period. The TOC APP will provide virtual clinical care for a wide range of diseases and across a wide spectrum of acuity. This position will require the ability to conduct a comprehensive post-discharge medical assessment, develop individualized care plans, and provide disease-specific clinical education regarding management and red flags. The TOC APP must also be comfortable diagnosing, managing, and monitoring decompensating patients with common readmission conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections. The role will require significant interdisciplinary collaboration with nurse case managers, clinical guides, and clinical coordinators, all aimed at providing the best post-discharge care for patients possible. The TOC APP will also help enhance the team culture of the program by fostering a safe environment for team communication and patient escalations. Provides specific instructions on each patient’s selected birth control method. • Triage phone calls to clinicians as appropriate. • Reviews electronic practice management (EPM) and electronic health records (EHR) systems for any alerts for outstanding follow-up needs and advise patients of their follow-up status or discuss the clinician’s recommendations with the patient at each visit. • Maintains an awareness of clinic flow and visit cycle times and contributes to the efficient delivery of care by working with a sense of urgency to respect the time of patients and increase access to services. • Maintains discretion and patient confidentiality at all times, using appropriate guidelines on release of information. • Contributes to a culture of continuous quality improvement by reporting incidents timely and completing incident reports. • Understands and complies with regulations from all governing entities and health center protocols including but not limited to Patient Rights, Child Abuse identification and reporting, infection control, laboratory protocols, emergency evacuation, risk management, and Occupational Safety and Health Administration (OSHA) safety standards. • Performs the following front office duties as assigned: o Greets and directs guests appropriately and in a courteous manner that projects a positive image of PPGT. o Receives and directs phone calls, schedule appointments, check-in patients, obtain necessary patient information to file and update patient records, and ensure all forms and consents are completed by patients o Supports the check in or check out areas. o Places reminder phone calls for scheduled appointments. o Visually inspects visitors/patients and packages entering and leaving the health center. o Observes security cameras and grounds, secures building at end of shift. o Completes daily health center closing procedures such as preparing deposits and balances cash drawer. Strictly adheres to revenue handling policies. o Reviews method of payment and obtains insurance information as applicable. • Maintains laboratory, sterilization, temperature and cleaning logs. • Handles lab specimens per organization protocols and performs laboratory testing as required by Clinical Laboratory Improvement Amendments (CLIA) when ordered by the clinician. • Sets-up and cleans laboratory area and exam rooms including maintaining a clean, neat arrangement; collecting and washing instruments; properly disposing of all trash; maintaining medical supplies and equipment in examination and laboratory areas. • Attends and participates in health center staff meetings and huddles. • Completes employee training for handling of situations involving conduct that endangers the welfare of minors, including mandatory reporting requirements. • Accepts individual and joint responsibility for, and the efforts to achieve health center annual goals regarding number of patients served, patient waiting time, appointment show rate, accuracy in the execution and recording of patient payments, customer satisfaction etc. • Has unrestricted access to patient protected health information (PHI) on paper and electronic forms health records for purposes of treatment, payment, and/or healthcare operations. The use of a patient’s protected health information should be limited to information needed for the specific task that is being performed or requested by the individual patient. All seasonal staff are required to work a minimum of 72 hours per pay period during the seasonal period. Regardless of start date, seasonal program/assignment ends on March 31, 2026. Within a close geography, you may be asked to float to another facility that has an emergent staffing need. Four (4) weekend shifts are required each 4 week schedule period. During the course of the 32-week period, the employee is required to work two (2) holiday shifts (defined by Department Manager or designee), if holidays fall during their temporary assignment period. Seasonal Staff are TEMPORARY status employees and are subject to first call-off. There are no guaranteed hours by the hospital. Seasonal Staff will not be eligible for benefits.
