Evaluates patients identified as “Very High Risk” (Braden Score 9 or less) per the Braden Report to determine if additional preventive care needed as well as validation of Braden care and interventions. Identifies patients at risk for pressure injury development, implements preventive care per established policies in collaboration with unit RNs and monitors response. Works effectively with other healthcare providers to assess, plan, implement, and evaluate patient care. Collaborates with multidisciplinary teams to assess patient needs for nursing care, patient/family education and other services.
