Keeps patient and resident plans of care updated reflecting changes in condition and responses to treatment or education Appropriately documents on patients and residents receiving treatments, reviewing previous documentation. Completes initial and Weekly skin Impairment forms Participates as an active member of the Interdisciplinary team, including the Wound Care Team for Weekly Wound Rounds and is part of the Wound Committee; completes the weekly wound report.
