Center Care Team - Medical Assistant, Center Float

Job Summary: Core to the ArchWell Health approach is to have a robust clinical care team focused on the needs of our members. Care teams are responsible for delivering excellent and compassionate primary care and coordinating the care of our members with the broader healthcare delivery system. The Medical Assistant (MA) plays a critical role in supporting the PCP day-to-day and ensuring efficiency in the care provided to patients. MAs will work closely and collaboratively with their teammates to greet and room ArchWell Health members. S/he will be expected to assist in assessing patient’s health conditions, including performing routine diagnostic testing during appointments. The MA-Float may be asked to provide coverage outside of their primary center. Duties/Responsibilities: Conduct intake with patients, measuring and recording vital signs, patient interview, and medical history; perform examinations and routine screenings, ensuring patients are informed and all questions and concerns are addressed for each procedure. Critically listen and engage with patients throughout appointment, ensuring needs and/or changing in patient conditions are reported to PCP, specialists, and/or other clinical personnel. Perform all tasks with a hyper focus on patient experience, including processing of patient phone messages, including returning calls; routing calls to other team members as appropriate; calling patients to obtain and relay pertinent information for the physician. Change dressings, apply bandages, remove sutures and other first aid procedures; use CPR skills, when necessary, all under physician supervision. Ensure compliance with OSHA safety standards and conduct accurate, legal, and ethical documentation at all times. Provide health coaching to a defined group of patients to support healthy lifestyle choices; follows up with coached patients via weekly calls. Required Skills/Abilities: Must possess a high degree of emotional intelligence and integrity, driven and focused work ethic. Self-starter with the ability to think creatively and work effectively. Ability to work effectively with various seniorities and diverse populations including staff, providers, members, family members, insurance carriers, vendors and market leadership. Strong communicator that is customer oriented. Excellent critical reasoning, decision-making, and problem-solving skills to analyze situations, determine risks, and find solutions to prevent future issues and resolve recurring defects. Experience with claims/quality reports and analytical software packages. Proficient skills in Microsoft Office Suite products including Word, PowerPoint, Outlook and Excel plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software. Ability to travel on occasion for company meetings. Education and Experience: High school diploma or equivalent education (GED) required. Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e. foreign-trained physician, paramedic, etc.) required. A minimum of 1 year work experience as a Medical Assistant preferred. 6+months' phlebotomy experience is highly desired. BLS for Healthcare Providers required. About this Job: General Summary of Position The Medical Assistant performs clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Position Summary We are seeking Licensed Practical Nurses (LPNs) to join our clinical team and support the medical needs of adult clients in residential substance use treatment. We are hiring for multiple full-time and part-time opportunities across evening and overnight shifts. The LPN plays a key role in client care through nursing assessments, medication administration, and collaboration with the broader clinical team. New graduates are encouraged to apply—we're happy to provide the training and support needed for success. Key Responsibilities Conduct initial nursing assessments for new admissions. Administer medications and monitor for therapeutic effects and side effects. Deliver care in alignment with detoxification protocols. Complete COWS and CIWA assessments in accordance with established protocols. Provide education to clients regarding medications and health conditions. Support medical providers during assessments and medication management appointments. Maintain accurate documentation while upholding strict confidentiality. Ensure medical equipment is functioning properly and safely. Support overall facility operations with professionalism and compassion. Perform other nursing-related duties as assigned. Qualifications Licensure: Active, unencumbered LPN license in the state of Massachusetts. Certifications: Current CPR/BLS, AED, and First Aid certification required. Experience: Previous nursing experience is preferred, but new graduates are welcome to apply. Training will be provided. Preferred: Experience working with individuals with substance use and co-occurring disorders and familiarity with detoxification protocols. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8. Basic computer skills to document in electronic medical records (EMR) 9. At least 18 years of age. 10. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
