Scheduled Weekly Hours: 40 Work Shift: Days/Afternoons (United States of America) Primary Function/General Purpose of Position The RN Clinical Quality Lead will be a clinical leader supporting the staff and providers in the medical group practice setting with a focus on improving quality of patient care and achieving key performance indicators. A ssists providers with monitoring their PC Dashboards and and managing gaps in patient care, including opportunities for outreach to improve Annual Wellness Visits completion. Works closely with the data analytics team and EPIC EHR team to optimize access to accurate and actionable quality performance data for use by the practice to improve patient care and services, population health management, and reduce the total cost of care. Facilitates daily care team huddles and pre-visit planning activities in the practice. Collaborates with the population health care team to leverage success with patient health maintenance goals and quality needs. Conducts nurse visits with patients, face to face and/or virtual to address preventative health topics, medication review, any quality care gaps and perform appropriate screenings. Identifies opportunities to improve Risk Score Accuracy through Hierarchical Condition Categories (HCC) documentation and facilitates education for providers toward success. Acts as a resource and directs activities for patient education and orientation to the practice. P articipates in regular practice meetings with providers and staff. Prepares and gives presentations in a professional manner. Attends required training, education and learning events by webinar, teleconference, virtual or in person.
