About Our Unit: Our CVOR is the heart of innovation at Atrium Health Carolinas Medical Center. We specialize in cardiac, vascular, and thoracic surgeries for both adult and pediatric patients. As a high-volume advanced heart failure center, we perform a high volume of LVAD, mechanical circulatory support and transplant procedures, offering nurses unparalleled experience in advanced surgical care. We are also a regional leader in pediatric cardiac surgery, providing life-saving interventions for our youngest patients. The Nurse Practitioner will play a critical role alongside a newly recruited Neuro‑Oncology physician under an established contractual agreement. JOB DESCRIPTION Job Summary Provides support for care management/care coordination long-term services and supports specific activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Completes comprehensive member assessments within regulated timelines, including in-person home visits as required. • Facilitates comprehensive waiver enrollment and disenrollment processes. • Develops and implements care plans, including a waiver service plan in collaboration with members, caregivers, physicians and/or other appropriate health care professionals and member support network to address the member needs and goals. • Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. • Promotes integration of services for members including behavioral health care and long-term services and supports (LTSS) and home and community resources to enhance continuity of care. • Assesses for medical necessity and authorizes all appropriate waiver services. • Evaluates covered benefits and advises appropriately regarding funding sources. • Facilitates interdisciplinary care team (ICT) meetings for approval or denial of services and informal ICT collaboration. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care and provides care coordination and assistance to members to address psycho/social, financial, and medical obstacles concerns. • Identifies critical incidents and develops prevention plans to assure member health and welfare. • May provide consultation, resources and recommendations to peers as needed. • Care manager RNs may be assigned complex member cases and medication regimens. • Care manager RNs may conduct medication reconciliation as needed. • 25-40% estimated local travel may be required (based upon state/contractual requirements). Short Description The Registered RN is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who supervises and provides care to these patients utilizing the nursing process including the therapeutic use of self. Follows established policies and procedures Communicates openly with internal customers and supervisors Provides excellent customer service interfacing well with all levels of staff, management and physicians Applies Cooper core values Other duties as required Overview The Surgical Care Unit (SCU) , Short Procedure Unit (SPU), Same Day Surgery (SDS) and Pre-Admission Testing (PAT) are in Camden at the hospital on the second, third and fifth floors. First Pre-Admission Testing, located in 3 Cooper calls to review a patient’s health information prior to a surgical procedure. Once approval for the recommended surgical procedure is finalized and patient information is reviewed the patient is provided with information on when and where to report. Once arrived and registered the patient is escorted to the appropriate location to be prepped for surgical procedure. These RN’s prep the patient for the procedure. They review medical history, medical procedures and place an IV. Patients are met by Anesthesia and their Surgeon. Once everything is reviewed and verified the patient then goes to the OR. Experience Required 1-year Critical Care Required (PACU preferred) Education Requirements Graduate of NLN accredited School of Nursing License/Certification Requirements Current NJ- RN License (Registered Nurse) Current BLS (Basic Life Support) Current ACLS (Advanced Cardiac Life Support) Obtained within 3 months of hire date: Current PALS (Pediatric Advanced Life Support) Preferred: Current CCRN (Critical Care Registered Nurse) or Current CPAN (Certified Perianesthesia Nurse) Special Requirements 930A-8P Weekend, holiday and rotation required
