MDS Coordinator Job Description Job Title: Minimum Data Set (MDS) Coordinator Reports To: Director of Nursing (DON) / Administrator Position Summary: The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) assessments, care planning, and compliance with federal and state regulations. This role ensures accurate clinical documentation, reimbursement optimization, and quality resident care in skilled nursing facilities and long-term care settings. Key Responsibilities MDS Assessment & Documentation Coordinate and complete MDS assessments according to regulatory timelines. Ensure accurate and comprehensive resident assessments. Review medical records to validate assessment data and supporting documentation. Submit MDS assessments to regulatory agencies within required deadlines. Monitor assessment schedules and maintain compliance with Medicare and Medicaid requirements. Care Planning Develop, coordinate, and update individualized care plans based on resident assessments. Facilitate interdisciplinary care plan meetings. Ensure care plans reflect residents' current needs, goals, and preferences. Communicate assessment findings to nursing staff, physicians, therapists, and other healthcare professionals. Regulatory Compliance Maintain compliance with Centers for Medicare & Medicaid Services (CMS) guidelines and state regulations. Participate in surveys, audits, and quality assurance activities. Stay current on MDS, RAI, and reimbursement regulations and updates. Educate staff regarding documentation standards and regulatory requirements. Reimbursement & Case Management Support Medicare and Medicaid reimbursement processes through accurate coding and assessment. Review documentation for appropriate Resource Utilization Group (RUG) or Patient-Driven Payment Model (PDPM) classification. Collaborate with billing and finance departments to maximize reimbursement while maintaining compliance. Monitor quality measures and identify opportunities for improvement. Quality Improvement Analyze quality indicators and quality measures. Participate in performance improvement initiatives. Assist with infection control, risk management, and resident outcome monitoring as needed. Qualifications Active Registered Nurse (RN) license; some facilities may consider LPN/LVN candidates. Certification such as RAC-CT preferred. Minimum 1–3 years of experience in long-term care, skilled nursing, or rehabilitation. Strong knowledge of MDS 3.0, RAI process, CMS regulations, and PDPM. Proficiency in electronic health records (EHR) systems. Excellent organizational, analytical, and communication skills. Certified Nursing Assistant (CNA) GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified Nursing Assistant (CNA) CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Behavioral Health RN: Assesses psychological needs of patient and families upon referral from nursing/medical personnel, external programs, and facilities. Provides discharge and aftercare planning as needed for a smooth transition from urgent care, inpatient, hospitalization, partial hospital program or intensive outpatient program. Represents psychiatric services to referral sources, community agencies, court services, social service agencies, and others as required. EP-RN Specialist : Assists physicians with procedures such as pacemaker, implantable defibrillator placement, angiograms, ablations, and other electrophysiology-based procedures; inserts cardiac catheters including central arterial/venous access catheters, performs catheter manipulation services. Operates and maintains all electrophysiology recording systems, stimulators and ablations systems, responsible for patient specimen testing, quality control and improvement. EP Coordinator: Oversees EP area and coordinates with Cardiac Cath Lab lead to better evaluate staffing and schedule needs, training required, materials management, and provides recommendations for efficiencies. Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication. Adheres to TMCH organizational and department-specific safety, confidentiality, values, policies and standards. Performs related duties as assigned. Minimum Qualifications EDUCATION : Bachelor’s degree preferred. EXPERIENCE : One year of nursing experience in an acute care setting preferred. (Positions assigned to EP: require two (2) years of EP experience or demonstrated competency in all areas of EP Lab procedures). LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) certification required. Additional certification may be required per department. Knowledge, Skills, And Abilities Knowledge of direct patient care and critical care procedures and techniques, tools, and responses required to ensure optimal patient care. Knowledge of exam indicators, contraindications, and patient preparation. Skill in communicating in a clear and concise manner with staff involved in critical care, and physicians to ensure the proper care of patients. Skill in reading 12-lead EKG. Skill demonstrated in assigned disciplines such as EP, directly involved in patient care. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to complete routine reports and correspondence. Ability to listen and accurately interpret others’ communication or instructions to take appropriate action. Ability to speak and communicate effectively. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables. NICU only: Ability to provide family centered developmentally appropriate care of newborns. Ability to handle the emotional stress associated with caring for a critically ill newborn or neonate. Ability to formulate a plan of care and support multidisciplinary collaboration.
