Our care team includes RNs and Nursing Station Technicians, with a nurse to patient ratio of 1:1 or 1:2. This is a 0.75 FTE position (60 hours every two weeks), day/night shift,12 hour shifts and every third weekend. Manages medication and infusion therapies. Provides care for patients undergoing diagnostic and therapeutic procedures/interventions on unit. Cares for critically ill, trauma and transplant patients and those who require observation and monitoring. Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Collaborates with the medical team to formulate a treatment plan to include care transitions and promote patient flow.
