Diabetes Program Nurse Practitioner/Physician Assistant - Sign on Bonus, Relocation, and Retention Bonus Eligible

Position summary: This is a grant funded position. Diabetes Program Nurse Practitioner is primarily responsible for providing medical management, care and education for persons diagnosed with diabetes and related chronic conditions, as well as prevention and wellness. Responsibilities JOB DESCRIPTION: The Milieu Manager provides leadership on the units; supervising and assigning daily tasks to MHS staff. Serves as a member of the interdisciplinary treatment team and supports the facility’s treatment program and philosophy.
