Responsibilities: Monitor Facility Quality Measures and analyze clinical data metrics to identify areas for improvement Ensure all MDS assessments are completed accurately, timely, and in full compliance with state and federal regulations. Opens MDS assessments, fills in resident's name, assessment reference date, and reason for the assessment. Reviews all validation reports from the State Department of Health and corrects errors as necessary. Transmits MDS data to State DOH after making certain assessments are edited and locked. Provides billing department with information regarding resource utilization groups for Medicare patients. Educates staff regarding MDS, Quality Indicators and Prospective Payment System. Assures regulatory compliance with MDS assessments. Assures MDS Encoders are following policy and procedure and entering data timely and accurately. Writes or revises policies and procedures for department. Provides in-service training regarding the MDS to Nurse Managers, RNs, LPNs, and other clinical staff (social service, recreation therapy, and dietitians). Report monthly progress to DON and supervise and oversee MDS process in doing care plan compliance Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures and create necessary policies and procedures to fulfill these gaps. Understands and adheres to the Medical Center’s Code of Conduct. Familiar with the Medical Center’s Mission, Vision, and Values Statements. Other Duties: Maintains connection with the DOH's MDS automation coordinator. Performs other duties as assigned to support the overall objectives of the department and organization. Position Qualifications: Five years or more as an MDS Coordinator. Meets regulatory requirements for MDS Prospective Payment System. Deep understanding of PDPM reimbursement model and CMS guidelines Computer literate. Good written communication skills. Good interpersonal skills. Speaks, reads and writes English to the extent required by the position. Prior management or supervisory experienced preferred Education: BSN Required, Master's degree preferred. MDS and PPS training required. RAC - CT is highly preferred Registered Nurse in State of New Jersey Job Setting /Physical Demands: Long Term Care department and related areas; contact with patients, visitors, staff. Frequent prolonged standing/sitting/walking. Overview Manages, coordinates, facilitates and supports the members of the interdisciplinary team in provision of hospice care to patient/families of VNS Health Hospice Care Program, including After Hours. Ensures the implementation of clinical/quality improvement initiatives for hospice services in compliance with regulatory requirements. Participates in the design, implementation, evaluation and modification of quality initiatives and educational plans. Initiates, coordinates, performs and assists in the analysis of clinical, regulatory and fiscal practice. Assists leadership in achieving Hospice Care goals, objectives, and fiscal targets by providing support with compliance and/or innovation to achieve improvement. Manages the day to day operations of the department, which includes effective caseload or visit management, achievement of productivity and revenue/expenditure targets for Hospice Care. Completes all work assignments, which includes audits, reports and projects. Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, and terminates staff and recommends salary actions as appropriate. Facilitates and manages the interdisciplinary group for provision of an effective and comprehensive hospice services to patient/families of VNSNY Hospice Program. Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a Registered Professional Nurse in New York State required Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred Education: Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field or the equivalent work experience required Work Experience: Minimum of three years clinical experience in hospice required Effective oral, written and interpersonal communication skills required Clinical management experience preferred Knowledge of personal computer operations, including MS Office applications preferred Pay Range USD $98,200.00 - USD $130,800.00 /Yr. Establish the facility’s policies and procedures to assure that key clinical personnel on all shifts are knowledgeable about the information found on the resident’s MDS+ and report changes on the resident’s status. 8. Coordinates with the Unit Coordinator and other disciplines the resident assessment (MDS+) and the care planning practices (RAPS). 9. Participates in the Interdisciplinary Resident Care Plan meeting to ensure that the activities of the committee result in proper identification of problems, complications, and risk factors. 10. Coordinates with the Director of Nursing the quarterly preparation and submission of PRI’s for RUG reimbursements. 11. Participates in departmental and interdisciplinary committees which influence or determining policies affecting nursing practice. 12. Coordinates the Employee Health Services in the Nursing Home. 13.Follows all appropriate safety procedures. 14. Respects residents’ rights at all time. 15. Performs all related duties as required and unrelated duties as requested during emergencies.
