Applies warm and cold compresses, consistent with facility procedure; performs irrigations, consistent with Facility procedure; performs other procedures related to skin care, as necessary; administers medications correctly and safely; correctly identifies medication in terms of action, dosage, side effects, and implications for the patient; meets standard on medication administration examination; demonstrates preparation of local solutions. Administers and documents medication correctly; educates patients and significant others related to drug and food interactions; educates patients and significant others related to drug and food interactions; participates in patient and family education; provides patient with an explanation of his/her condition as indicated; communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; provides for early discharge planning and appropriate referrals for post-hospital care; evaluates the effectiveness of teaching by feedback from patient/ family and documents same. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. Functions as a respectful member of the health care team. Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities. Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MDs etc. Provides consultation about the therapeutic regimen to the nursing staff responsible for the patients care in the hospital setting. Works collaboratively with nursing leadership to ensure timely communications with the nursing staff. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, housestaff, other disciplines and ancillary services. Participates in the development and revision of interdisciplinary and nursing standards, patient and family education materials as relevant to area of practice and expertise. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members. Minimum Qualifications: To qualify you must have: Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York. Education: Master's Degree in Nursing. Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice. Basic Life Support Certification through the American Heart Association.. Required Licenses: Registered Nurse License-NYS, Nurse Practitioner-NYS, Basic Life Support Cert Preferred Qualifications: Advanced Life Support certification (ACLS, PALS) through the American Heart Association or Neonatal Resuscitation certification (NRP) through the American Academy of Pediatrics may be required based on your selected privileges. At least five years of clinical nursing practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and satisfaction. Previous experience as a nurse practitioner is preferred. Evidence of excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership; previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team; Effective change agent. Overview Provides high quality nursing care and personal care to patients under the direction of the registered professional nurse consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Provides nursing care to patients in any of the service areas. Works under moderate supervision. • Utilizes skills of observation, interviewing and physical/psychosocial assessment to gather relevant data in identifying changes in condition, and assesses patient living conditions and family situations, alerting the registered professional nurse or coach, as appropriate. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Delivers nursing care to patients in any of VNS Health’s services/settings. • Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations. • Participates in the development of treatment plans and interventions with the registered professional nurse. • Assists the registered professional nurse in carrying out the care plan and performing specialized duties when necessary. • Provides health instructions to patients and family caregivers in accordance with the registered professional nurse’s plan of care. • Performs technical procedures safely, efficiently, legally and within the scope of practice. • Documents assessment findings; treatment and interventions provided with patient/family responses in accordance with VNS Home Care policies. • Assumes responsibility for personal professional growth. • Understands and interprets agency reimbursement policy. • Collaborates with teammates in setting of team goals, establishment of performance measures, and evaluation of team performance. • Participates in activities that support achievement of team goals and responsibilities. • Participates in coordination of day to day service delivery operations of team (e.g. coordination of patient coverage during planned and unplanned absences of teammates. • Demonstrates awareness and appreciation of roles and contributions of individual team members. • Participates in peer review with other team members and coach support. • Participates in the orientation of new team members. • Serves as team representative for special events and projects, as requested. • Participates in team meetings and other team activities. • Shares accountability with team members for achieving team performance goals. • Performs other duties, as needed, including personal care services. • For CMHS only: • Patient assignments are for routine long acting injections and medication delivery. • For Hospice and Home Care only: • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Participates in special projects and performs other duties as assigned. However the nurse will be cross–trained in each service area and will be expected to perform nursing care duties in all three Hospice services, as needed.
