The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. Expect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. Union and surrounding counties, NJ Primary Responsibilities Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs Develop and implement person centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management and member safety in alignment with evidence-based guidelines Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan Provide education and coaching to support member self-management of care needs and lifestyle changes to promote health Support proactive discharge planning and manage/coordinate Care Transition following ER visit, inpatient or Skilled Nursing Facility (SNF) admission Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents. RESPONSIBILITIES: Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift Makes rounds on assigned residents at least once every two hours Administers medication and treatments according to the physician's orders. Assumes responsibility for ordering medications Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes Assists the attending physician with rounds and special procedures as needed Transcribes, documents and implements physician's orders Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Such evaluations become part of the nursing assistants’ permanent records and shall be used as the basis to determine the continued employment of nursing assistants, promotional opportunities and the amount of any wage increases granted Assumes independent responsibility for documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and recommending the suspension or discharge of an employee Assumes independent responsibility for completing required routine documentation and auditing as required Participates in the hiring, interviewing and orientation of nursing assistants Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: Practical Nurse licensed in the State of employment Minimum one year of education or experience in geriatric nursing or rehabilitation preferred Knowledge of nursing theory and practice, including the administration of medications Ability to read, write and comprehend English CPR certified SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities. OVERVIEW: Assist the Director of Nursing and Assistant Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: • Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly • Assesses residents/clients upon admission and periodically as conditions change • Performs rounds to evaluate patient condition • Provides or assists with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated • Obtains and implements Physician Orders, checking for accuracy and completeness • Communicates between shifts regarding changes in resident/patient conditions • Ensures a safe and secure environment for patients/residents, staff and visitors • Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients • Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. • Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services • Participate in developing, implementing, and periodically updating the resident’s written discharge plan • Ensure that a sufficient number of nursing staff is available for each shift to ensure that quality care is maintained • Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director of Nursing Services • Review complaints and grievances made or filed by department personnel.
