A Brief Overview A clinically competent practitioner of pediatric, family centered nursing. Provides safe nursing care to the general division population with minimal guidance, and bases practice on identified principles and standards of care. Responsible for day-to-day patient care, assessment and treatment in an outpatient office and comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. About the Division The Subspecialty Practice Nurse will support the outpatient clinic to ensure patients are seen in a timely manner, provide patient education and care coordination. The Subspecialty Practice Nurse will work primarily in the Princeton Specialty Care Center but will be expected to travel to other locations if needed including Voorhees and May Landing in New Jersey. There are also Specialty Care Centers in Chalfont, Abington, King of Prussia and Brandywine Valley as well as the Buerger Building on CHOP's Main Campus. During orientation you will be asked to go to other locations besides Princeton NJ so there is a fair amount of travel required during those first 90 days What you will do Practice Follows Nursing Process to deliver safe care: assessment, planning, implementation, and evaluation. Synthesizes patient assessment data and recognizes complex situations prior to communicating with care team. Collaborates with family, team members and other caregivers to initiate/update plan of care, patient/family education. Manages care of complex patients utilizing proper prioritization and delegation to appropriate staff. Identifies expected outcomes and revises plan of care when unexpected outcomes arise and begins to use full range of communication as a means of conveying plan. Documentation reflects ongoing evaluation and effectiveness of patient care utilizing current standards, policies and procedures. Provides care that meets professional standards of practice and meets regulatory requirements. Communicates and coordinates with providers or Care Team regarding hospital discharge, Emergency Department visits, consulting physicians, community resources, disease management, patient self-management, patient and/or family education on chronic conditions, and labs or other testing as well as specialist visits. Actively participates in department activities; attends departmental meetings; actively participates as a team member in resolution of problems as they are identified; analyzes current procedures. Responsible to dispose Bio-Hazardous waste and regular waste per practice protocol. Maintains vaccine temperature logs, inventory and ordering of medical supplies (i.e.: vaccines, medications), disposing of expired sample medication, and stocking rooms to set par levels. Performs other administrative tasks (e.g. check-in and check-out functions, opening and closing office procedures, etc...) as needed. Brings suggestions for improvement to the attention of Practice Managers and Providers for consideration. Actively participates in maintaining and/or improving quality improvement initiatives. Occasional requirement to lift up to 25 pounds. Occasionally push wheelchairs with patients weighing up to 325 pounds. EDUCATION: High School degree or equivalent required. TRAINING AND EXPERIENCE: One year of clinical experience OR graduate of Medical Assistant program required. Two years of experience in similar healthcare setting is preferred. A portion of the unit, the Jim Craigie Center for Joint Replacement, is a dedicated use area for total hip and knee replacement patients. The Surgical Care Unit is certified as a Bariatric Center of Excellence and Joint Commission Advanced Hip and Knee Replacement Center. Key Roles and Responsibilities: Nurses on SCU will provide pre and post-operative care for patients recovering from a variety of surgical procedures (hip fractures, spinal surgery, other orthopedic surgeries, bariatrics, various abdominal surgeries, breast surgeries, urological interventions, etc.) Emphasis on patient and family education to ensure clear expectations and optimal outcomes during surgical recovery, both in the hospital and at home Some of the clinical skills performed on a daily basis consist of: safe patient ambulation, pain management, drain care, nasogastric tube care, foley catheter care, chest tubes, central lines, wound vac and wound care, complex medication administration, blood administration and constant communication with the interdisciplinary team.
