Nurse Next Door affiliated home care aides shall be selected on the basis of such factors as a sympathetic attitude towards the care of the sick, ability to carry out directions, maturity and ability to deal effectively with the demands of the job. All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. Job Description Join a highly skilled, dynamic group of APPs in the outpatient clinic in our Hematology, Blood & Marrow Transplant and Cellular Therapies department at the Parnassus campus of UCSF Health. Our acclaimed program draws patients from around the world seeking effective and innovative therapies to treat their underlying blood disorders or blood cancers. Our patient population includes those receiving chemotherapy, stem cell transplants, or cellular therapies for the treatment of hematologic malignancies. In this position, the APP would work in the outpatient clinic 4 days a week for 10 hours a shift. They will primarily act as an independent billing provider, seeing follow-up visits and hospital discharges. There may be opportunities to incorporate new patient referrals and survivorship visits into your day. The APP will have the opportunity to learn and perform various procedures, including bone marrow biopsies and lumbar punctures. Responsibilities Evaluates and manages patients in independent APP encounters, including telehealth. Includes providing care to new, follow-up, acute, routine, and specialty patients. Includes taking histories, performing physical examinations, interpreting diagnostic tests, formulating evidence-based assessments and plans, responding to changes in patient status, and ordering medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. Reviews or collaborates with attending physician as needed. Care provided must be documented in the electronic medical record in accordance with the UCSF Rules and Regulations . Provider services must be billed to reflect the performing provider and comply with the APP Billing Policy 3.08.01 . 1.2. Duties: Assist clients with activities of daily living, such as bathing, dressing, grooming, and toileting Support clients with mobility challenges and provide assistance with lifting when required Offer companionship, emotional support, and engage in meaningful interactions Ensure a clean and safe living environment for clients Assists in meeting nutritional needs by providing meal planning, preparation, and grocery shopping as needed Provide clients medication reminders Support clients on walks and/or light exercises Monitor and promptly report any changes in clients' conditions or behaviors Skills: Prior experience in caregiving, senior care, or related fields preferred (professional or personal) Experience working with individuals requiring specialized care (memory care, Parkinson's disease, cancer care, hospice, etc.) is a plus Compassion and empathy towards others Proficient verbal and written communication skills Ability to work independently while following instructions accurately Strong organizational abilities with attention to detail Capacity to adapt calmly and effectively to changing situations Requirements: Home Care Aide (HCA) Registration with the State of California (Or ability to obtain) Proof of LiveScan Fingerprint (or ability to obtain) Proof of current TB testing (Or ability to obtain) Clear a criminal background check (National and required by state & county) Clear Guardian Professional and Personal Reference Check Must be physically able to perform the functions of this position Have reliable transportation, a valid driver's license, and proof of sufficient automobile insurance (when applicable) Benefits: Competitive pay ($20/hr+) Lots of opportunities to earn bonuses: Loyalty Bonus Program, Perfect Attendance Bonus Program, Referral Bonus Program (and more!)
