Adheres to infection prevention and control policies and procedures. Adheres to high level of disinfectant, point of use, peel packs policies, and procedures. Perform related patient care functions such as taking temperature, pulse, respiration and blood pressure readings, wound care, may instruct patient in various self-administered treatment or procedures; obtain specimens; assures correct culture collection. Assist physician/advance practice providers or other medical staff personnel with examinations and treatments by selecting and passing instruments, providing support and restraint to the patient, draping, and other assistance as directed or required. Within scope observes and documents patients condition by gathering signs, symptoms, and reactions to therapy, to identify unexpected and/or significant findings, initiates appropriate emergency procedures as necessary, and reports unusual conditions to the registered nurse/advanced practice providers as established by policy and procedure in a timely manner. May volunteer to start and maintain intravenous flow rate with Certifications, notifies registered nurse/advanced practice/physician of any problems. May discontinue IV therapy except intra-cath and central venous lines. May volunteer to start perform venipuncture/blood withdrawal with Certifications. Collects and processes specimens per policy, procedures, guidelines, and protocols. Provides non-compoicated teaching following the registered nurse/advanced practice provider/physician plan for education. Report any identified learning needs and discharge barriers. Ensures cleanliness of nursing module/station, patient exam room, and proper functioning of patient care equipment by visual inspection, routine testing, and other related methods. Provides a safe and therapeutic environment for patients and staff. Utilizes effective verbal and non-verbal forms of communication. Performs in other departments or modules to which assigned as required by the nursing/department administration to provide and maintain the established standards of care and safety. Performs departmental back-office workflows including but not limited to proactive office encounter, successfully closing care gaps, patient outreach, access and/or nurse visits. Participates in staff meetings, and in-service programs. Assists in ordering and distribution/stocking of module/department supplies and equipment. Accepts verbal and telephone orders from the physician/advanced practice providers. Supports patient communication and message management including but not limited to patient emails, call center messages, and all other back-office messages. Administers pulse oximetry, humidification therapy, spirometer, oxygen therapy via mask or nasal cannula, oral, nasal, and trach suctioning, and sputum induction. Documents care in Health Connect per policy, procedures, guidelines, and protocols. Performs other related duties as assigned and required, including orientation of new employees. Assists in coordinating the discharge process. Performs all job functions within license certification and scope of practice. Promotes, ensures, and improves customer service to internal/external customers by demonstrating skills which are consistent with the organizations philosophy of providing extraordinary customer relations and quality service. Job Description The University of California, San Francisco (UCSF) Advanced Heart Failure Comprehensive Care Center’s Mechanical Circulatory Support (MCS) Program seeks a full-time ECMO Specialist Nurse to help grow our program. The AHFCCC team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, researchers and others. Our MCS program is nationally known for cutting edge MCS support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The ECMO Nurse Specialist is a highly skilled critical care nurse responsible for the initiation, management, and monitoring of Extracorporeal Membrane Oxygenation (ECMO) support for critically ill patients. Working as part of a multidisciplinary team, the ECMO Specialist ensures safe, effective circuit operation, timely response to complications, and adherence to established clinical protocols in order to optimize patient outcomes. The Extra Corporeal Membrane Oxygenation (ECMO) Nurse Specialist will be responsible for providing device related care to patients on ECMO as well as other temporary and durable MCS devices in the inpatient, operative and inter hospital setting. Qualifications REQUIRED QUALIFICATIONS A minimum of four (4) years recent clinical RN experience working in adult critical care setting (ED, ICU). A minimum of two (2) years recent RN experience working with ECMO patients. A minimum of one (1) year experience working with VAD patients. Strong understanding of cardiopulmonary physiology, hemodynamics, and coagulation management. Ability to interpret complex clinical data and respond quickly to changing patient conditions. Job Description BCH Oakland Day Hospital Infusion Center serves the Specialty populations at UCSF Benioff Children’s Hospital. The treatments include but are not limited to apheresis, chemotherapy, biotherapy, immunotherapy, blood transfusions, hydration and electrolyte therapy, injections and blood draws, enzymes and iron infusions, and antibiotics. The center consists of 14 beds, and treatments include research and standard of care infusions. The infusion centers are staffed by Nurses, Medical Assistants, and a Practice Coordinator. This position requires the incumbent to utilize the nursing process for assessment, planning, implementation and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with complex needs. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.

Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. We Provide paid training and help you with the registration process. · Completion of a criminal background check and disclosure of any criminal history or evidence of exemption. · Evidence of entry-level training or plan to complete paid training prior to client care and in accordance with CA regulations. · Evidence of tuberculosis testing and verification that the home care aide does not have active tuberculosis. · Ability to read and follow written instructions and document care given. · Self-directing with the ability to work with little direct supervision. · Empathy for the needs of the ill, injured, frail and the impaired. · Demonstrates tact, patience, and good personal hygiene. · Valid driver’s license and auto insurance and/or reliable transportation required. Physical Demands · Regularly required to sit, use hands or fingers, handle, or feel and talk or hear. · Required to reach with hands and arms. · Required to stand, walk, climb or balance, stoop, kneel, crouch, or crawl, and taste or smell. · Must be able to lift and or move up to 10 pounds, frequently lift and or move from 25 up to 50 pounds. · Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
