The North side houses 37 patients, and the South side houses 36 patients. There are two negative pressure rooms with an anti-room. There is a pharmacist present on the unit to assist the RNs with medication questions/concerns. Multidisciplinary rounds occur every Monday through Friday. Rounds involve all aspects of the healthcare team, including the patient and family. Overview The Clinical Decision Unit is on the first floor of the Kelemen building. We are a locked unit and require a badge to enter or ring the doorbell to be let in. The Clinical Decisions Unit (CDU) is a35-bed, single-room unit.The patients are typically here for observation or if needed to be admitted as an inpatient they will stay in CDU until a floor bed is available. Multidisciplinary rounds are done Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. The Express Admit Unit (EXAU) is under the management of the CDU. There are 6-bed areas where the admitted patients stay until a floor bed is available. This gives them privacy and care while waiting for their room to be available. Both CDU and EXAU can have patients on telemetry for cardiac monitoring. Purpose: We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases. What You'll Do: Provide high quality care to primary and palliative patients in and around Monmouth County, NJ Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care. Effectively communicate with office staff and clinical operations across various internal platforms Provide high quality, patient centered care to a panel of patients ` Attend quarterly medical staff meetings Develop impressions, diagnosis, and treatment plans Foster trust and consult with patients, families, and caregivers during visits Order diagnostic testing as medically necessary Prescribe medications supplies and appropriate medically necessary interventions What You'll Need: Board Certified, with a valid NP certification in the state of New Jersey Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose Positive demeanor and strong people skills Ability to confidently problem solve and operate independently 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required) What you'll get: A flexible schedule - so you can see your patients during a schedule that works for you Autonomy - so you can feel trusted to provide the best care possible.
