Job Description: Care Manager – Registered Nurse Monogram Health is looking for skilled Registered Nurse eager for the opportunity to make a difference in patients' lives. The Care Manager RN is a key member of an integrated Care Team which includes an Advanced Practice Provider and a Social Worker. The patients we serve often struggle with multiple serious diseases. Registered Nurses help patients improve their quality of life in the home and slow the progression of kidney disease, enabling positive health outcomes. While the full range is posted, most offers will typically fall within a narrower band based on these factors. In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package. Responsibilities Work closely with patients’ medical providers to develop and continually adapt care plan Perform in-home care management visits to execute care management plan Monitor biometric data and follow approved protocols for any necessary interventions Inventory and reconcile medications and coordinate with pharmacists and prescribers Perform patient health assessments and surveys as required Deliver individual and group education on CKD, ESRD, dialysis and associated comorbidities Encourage medication and treatment adherence through frequent contact with patients Engage family and social support groups in the education and care of patients Serve as the primary point of contact and be the first call when patients have questions (business hours) Provide education and coaching around medications, medical conditions, diet, exercise, and lifestyle choices Educate patients and facilitate conversations around proactive care decisions, especially relating to Advance Care Plans and ESRD treatment modalities Obtain vital signs when visiting patient and escalate any concerns to the provider Initiate patient relationships through enrolment and onboarding processes Perform post-op and hospital discharge visits to help patients through vulnerable transitions Review and document patient updates and progress in care management platform Coordinate with dialysis providers to ensure transitions of care are seamless Position Requirements Frequent local travel to perform in-home visits Basic Life Support (BLS) certification is required in this role. Responsibilities Include: Essential Duties and Responsibilities Initiates the admission process by obtaining provider orders upon patient arrival at a residential facility and completing the initial nursing assessment within 24 hours, including assistance with medical intake (patient history and physical). Performs vital sign monitoring, including orthostatic vitals and blind weights as ordered by the provider; conducts patient assessments and documents findings in detailed nursing notes. Monitors and documents patient progress or regression in accordance with established policies and procedures. Assists providers in facilitating weekly patient consultations. Offers education and support to help patients develop the skills needed for safe self-administration of medications after discharge. Ensures accurate placement of medications in the designated medication box and updates the medication administration record (MAR) accordingly. Supports patients in obtaining medications from the pharmacy, including facilitating initial medication orders. Coordinates and facilitates the collection of laboratory specimens as ordered, ensures timely receipt of results, and communicates findings to the medical team for appropriate follow-up. Other Duties and Responsibilities Demonstrates knowledge of medication actions, side effects, and contraindications, and is responsible for researching unfamiliar medications prior to administration to ensure safe and informed care. Collects and records data in a comprehensive, accurate, and systematic manner. Responsible for maintaining patients’ records updated in electronic system. Supports a therapeutic milieu by monitoring patient behaviors, fostering a recovery-focused environment, and collaborating with the treatment team to reinforce structure, safety, and consistency. Communicates relevant information obtained from patients or families to the interdisciplinary team to support individualized care planning and treatment continuity. Coordinates with the Primary Therapist on any insurance review information needed. Works collaboratively with medical and psychiatric teams to provide integrated nursing care in support of eating disorder treatment goals. Participates in discharge planning with the medical and psychiatric teams to ensure continuity of care and safe transition from eating disorder treatment. Attend monthly nurse-led meetings and other team meetings as they arise to support effective communication and collaboration. Provides health education to patients through individual or group sessions when appropriate and documents sessions clearly, accurately, and concisely using the designated form. Assist patients in the coordination of their outpatient appointments as needed. Supports the informed consent process by helping patients understand procedures and treatments under the direction of the provider. Knows Baker Act (Florida Only) Delivers clinical nursing care in a safe, competent, and professional manner in accordance with established standards and protocols. Adheres to established standards of nursing practice and organizational policies. Completes required onboarding and annual training within designated timeframes and attends monthly clinical services training or other training as scheduled, in accordance with organizational policies and procedures. Demonstrates professionalism and serves as a positive role model for patients and staff within the therapeutic environment. Promptly reports suspected patient rights violations by established protocols.
