Psychiatric Mental Health Nurse Practitioner - Adult Intensive Outpatient Program (IOP)

Greenwich Hospital's Department of Psychiatry is seeking an experienced Advanced Practice Registered Nurse (APRN) to support the Adult Intensive Outpatient Program (IOP) located at 2015 West Main Street. The Adult IOP serves patients with high-acuity behavioral health needs who are transitioning from inpatient psychiatric hospitalization to a less restrictive level of care. This role is integral to the interdisciplinary treatment team and is responsible for providing comprehensive psychiatric evaluation, medication management, and ongoing clinical support to patients participating in the program. The successful candidate will work collaboratively with psychiatrists, therapists, nursing staff, and other behavioral health professionals to promote stabilization, recovery, and successful community reintegration. Essential Responsibilities Provide psychiatric assessments and ongoing evaluation of adult behavioral health patients enrolled in the IOP. Manage psychotropic medications, including prescribing, monitoring effectiveness, and adjusting treatment plans as clinically appropriate. Develop and implement individualized treatment plans in collaboration with the multidisciplinary care team. Monitor patient progress, symptom management, and treatment outcomes. Provide patient and family education regarding diagnoses, medications, and treatment recommendations. Coordinate care with inpatient psychiatric units, outpatient providers, primary care clinicians, and community resources. Ensure timely and accurate documentation in accordance with regulatory, organizational, and departmental standards. Participate in program development, quality initiatives, and interdisciplinary case reviews. Qualifications Required Active Connecticut APRN license. Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or equivalent psychiatric APRN certification. Minimum of 1-2 years of independent clinical practice experience in behavioral health or psychiatry . Demonstrated experience with psychiatric assessment, diagnosis, and medication management. Strong communication, collaboration, and clinical decision-making skills. Preferred Experience working in intensive outpatient, partial hospitalization, inpatient psychiatry, or other acute behavioral health settings. Experience caring for patients transitioning from inpatient psychiatric hospitalization to outpatient treatment. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. Patient Education & Psychosocial Support * The NP/PA will educate patients/families on care and recommended or available resources. In collaboration with the supervising/collaborating MD, this may be inclusive of admitting instructions, treatment plan, actual and potential side effects of treatments/medications, and discharge plan. * The NP/PA, as part of a multidisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner. 3. Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure. 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations. 5. The Susan D. Flynn Pediatric Oncology Nursing Fellowship program is intended to help stimulate the career interest and foster the professional development of potential Pediatric Oncology Nurses. The program is focused on the critically important field of Oncology Nursing. The Fellowship enhances the Student Nurse Intern's core clinical education through direct involvement with Oncology patient care, expert training in compassionate care, and exposure to related patient care duties and services that typify Oncology nurses' valuable role and important inter-relationships with other members of the healthcare team. Opportunities to attend various pertinent internal educational programs and meetings with key team members at Connecticut Children's Medical Center provide additional learning experiences for the Student Nurse Intern.
