Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Job Description Nemours is seeking a full-time Registered Nurse to join our Nemours Children's Health team in Wilmington, DE. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Position is full-time, rotating shifts, 60 hrs in 2 wks, 12-hr shifts, 2/6 weekends, every other holiday. The primary role of the Registered Nurse is to implement the patient care delivery model for the patient and their family. Additionally the registered nurse is responsible and accountable for assessing, planning, providing and evaluating, the care provided to assigned patients. Additional Requirements Must be a graduate of an accredited school of nursing. BSN Preferred, obtained within 3 years of start required Inpatient Experience Preferred Must be eligible for Delaware State license (or compact license). This Hiring manager prefers RN with previous Onco and Chemo administration experience. 3 East Oncology: Bone Marrow Transplant BMT is a 26 Bed unit specializing in the care of Oncology and Stem Cell Transplant patients. Care for all ages -infancy to early adulthood. Common diagnoses treated on the unit include Leukemia, Lymphoma, Solid Organ Tumors, Neuro Oncology disease processes which may result in Stem Cell Transplant. The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care. The Clinical Nurse adheres to the organizational and nursing department mission, vision, and values. Clinical Nurses practice in accordance with the ANA Code of Ethics; the ANA Standards of Clinical Nursing Practice; practice standards of applicable specialty nursing organizations; and CHOP developed standards of nursing practice. The professional nurse practices within the defined scope of practice designated by the Nurse Practice Acts of Pennsylvania and/or New Jersey, depending on the assigned work site. What you will do Clinical Practice Follows the nursing process to deliver safe, compassionate, and culturally effective care. Gathers and recognizes/synthesizes assessment data to determine the patient/family plan of care, pertinent data to be shared with care team, and tasks to be delegated. Recognizes and/or seeks resources to recognize clinical changes that are indicative of patient deterioration; utilizes the CAT team when appropriate. Collaborates with the patient, family and multidisciplinary team to coordinate all facets of patient care. Develops, implements and/or revises the patient’s plan of care on admission and during the course of the hospitalization.
