Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor. Job Description: Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. Troubleshoots/declots obstructed central line catheters. Monitors PICC/Midline and educates on prevention of line infection and routine care. A nursing assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected. 
As an integral part of the interdisciplinary team, the Registered Nurse utilizes the nursing process to provide therapeutic care to a specific population of patients. The perioperative registered nurse uses the nursing process, designs, coordinates, and delivers care to meet the identified needs of all patients during perioperative phase. Accurately documents patient charges, implant records, pathology specimens, and preoperative record in a timely manner. Accurately utilizes Cerner system in documentation, updates preference cards as needed. Adheres to policy for instruments/sponges/sharps count. Practices according to USCUH collaboration principles and demonstrates respectful teamwork and delegation. Follows policies, Title 22 and JCAHO Standards. Participates in development of professional practice, i.e., Nursing committees, research, policy development/review, unit based improvement projects Demonstrates knowledge about current policies and shares awareness about new trends/technology. Demonstrates current knowledge of surgical anatomy and surgical procedures and applies that knowledge in preparing equipment, supplies and instrumentation for assigned procedures. Maintains specialty carts on a continuous basis. Supplies are maintained at par level, organized, cleaned, outdated. Assigned staff is a resource to others for the specialty cart. Surgical suites are organized and prepared daily (OR, run room, hallway, carts) Facilitates the smooth flow of the surgical intervention (i.e., prepares for the procedure with updated preference cards, knowledge of instruments and Equipment) Participates in hospital and departmental wide PI activities Performs other related duties as assigned.
