A Brief Overview Children's Hospital of Philadelphia is seeking a Licensed Practical Nurse for their Chadds Ford Primary Care office. This position will be Monday-Friday and will require one evening shift per week and a rotating Saturday shift. The shift will be based off the operational need. Two years of LPN and pediatric experience is preferred. The Licensed Practical Nurse performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. The LPN participates in the planning, implementation and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate if needed to other Care Network sites within the designated cluster. What you will do Provide and document direct nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention and modification of the plan of care. Assist other health care personnel in the delivery of patient care. Participate in maintaining the environment of care including equipment and other material resources. Participate in own professional development by maintaining required competencies and attending educational offerings. Supports the development of other staff and formal learners. Participate in the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities. The LPN may administer medications that are with their scope of practice and organizational standards only if the following conditions are met: a) the LPN has received and satisfactorily completed a Board approved educational program which requires study and supervised clinical practice intended to provide training necessary for administering immunizing agents. b)A written order has been issued by a licensed physician pertaining to an individual patient or group of patients, c) the LPN has knowledge of the health system policies and has met competency training standards. Support clinical activities related to nursing and clinic flow and make recommendations to manager for improvement in quality and effectiveness of nursing care given. Demonstrate commitment to patient safety by speaking up about safety issues and changing nursing practices to enhance safety. Share errors through system reporting mechanisms, and demonstrates shared accountability for safe practices. Perform other related duties incidental to the work described herein. QUALIFICATIONS: Currently certified in New Jersey as a Certified Nursing Assistant. Ability to read, write and comprehend English. Long-term care experience preferred. POSITION SUMMARY: Under the supervision of a licensed RN or LPN performs nursing services based on the facility’s policies and procedures as regulated by the State of NJ, DOH and Federal regulations. Provides assistance to residents with daily routines and other resident activities as needed. PART I: JOB RESPONSIBILITIES AND STANDARDS 1. Follows the plan of care. Attends IDCP meeting as assigned. 2. Demonstrates proper body mechanics. 3. Provides personal care to residents/guests (bathing, cleaning finger nails, shaving and perineal care, etc.) and assures that resident/guest dresses appropriately. 4. Follows infection control procedures (i.e. hand washing/sorting of linens) 5. Performs routine /special skin care as indicated. Reports skin alterations immediately to nurse. 6. Provides privacy for residents and adheres to HIPAA standards. 7. Inquires about residents/guests comfort/discomfort level on a scale of 0-10. 8. Provides restorative care as indicated. 9. Performs routine catheter care 10. Provides care to residents/guests with tubes and special equipment. 11. Transports resident/guest to and from activities. 12. Answers all call bell and places them in reach of resident/guest. 13. Fills water pitchers with fresh ice water unless contraindicated. 14. Offers snacks to all residents/guests and documents appropriately. 15. Monitors residents during smoking periods when assigned. 16. Demonstrates ability to deescalate resident through verbal strategies, redirections and crisis management and report to nurse immediately. 17. Assist with meal pass and feeding in a timely manner. 18. Follows Fall Prevention Protocol. 19. Collects lab specimens. 20. Reports condition changes/behavioral issues to nurse immediately. 21. Follows elopement risk protocol. 22. Maintains cleanliness and organization of resident/guest rooms including bed making. 23. Accompanies resident/guests during outside medical consultations and out of building activities. 24. Follows safe-handling procedures for personal safety and resident safety at all times. 25. Accurately reflects resident needs on ADL Tracker. 26. Other duties as assigned. PART II. DOCUMENTATION DOCUMENTS THE FOLLOWING: 1. Intake and output/hydration. 2. Food intake/calorie count/snack. 3.
