Overview • Department: Acute Rehab • Schedule: Full-Time, Nights, 7p-7a, weekend and holiday rotation • Facility: Prime Healthcare Saint Joseph Hospital • Location: Elgin, IL Responsibilities The Patient Care Assistant assists patients with activities of daily living, provides basic nursing care to all patients and assists in maintenance of a safe environment under the direction and supervision of the Registered Nurse in charge of the team and/or unit. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. High School Diploma or equivalent. 2. Certified Nursing Assistant (CNA), Illinois approved Certification required. 3. Current Phlebotomy certification upon hire, preferred. 4. Current EKG certification upon hire, preferred. 5. Current BLS (AHA) within 30 days of hire and maintain current. 6. Possesses knowledge of Medical Terminology. 7. Minimum six months previous experience in an acute care hospital or physician’s office preferred. Qualifications: Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings. Bachelor's degree strongly preferred. Leadership or management experience a major plus. Valid driver's license (required) Responsibilities: Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more. Ensure adherence to hospice regulations and standards. Facilitate communication between medical staff and the care team. Lead interdisciplinary meetings, documenting patient needs and care plans. Develop and update patient care policies and emergency procedures. Manage staff scheduling, performance reviews, and foster a positive work environment. Act as the central hub for communication with families, physicians, and other providers. Analyze quality metrics and manage a caseload of patients as needed. Participate in after-hours and on-call duties to ensure seamless care. Maximize resource utilization and promote a collaborative work culture.
