About the Opportunity This unique role combines outpatient valve clinic management with inpatient structural heart care coordination. The APP will participate in all aspects of the patient journey, including initial consultation, diagnostic workup, multidisciplinary Heart Team evaluation, procedural planning, hospitalization, discharge coordination, and follow-up. Key Responsibilities Outpatient Structural Heart & Valve Clinic · Evaluate and manage patients with severe aortic , mitral, and tricuspid valve diseases, as well as other structural heart conditions. · Participate in new patient consultations and follow-up visits. · Coordinate diagnostic testing, imaging reviews, and procedural workups. · Participate in multidisciplinary Heart Team conferences. · Manage pre-procedure and post-procedure care. · Collaborate with referring physicians and community cardiologists.
