Required Qualifications Active and unrestricted RN Compact licensure . Minimum 3+ years clinical practical experience preference: (Diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members. Minimum 2+ years Case Management, discharge planning and/or home health care coordination experience. ​Preferred Qualifications Excellent analytical and problem-solving skills. Effective communications, organizational, and interpersonal skills. Ability to work independently (may require working from home). Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications. Efficient and Effective computer skills including navigating multiple systems and keyboarding. Willing and able to obtain multi state RN licenses if needed, company will provide. Certified Case Manager is preferred. Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) is preferred, but not required. Education Associates degree or Nursing diploma require. Bachelor’s degree preferred. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Key Responsibilities: • To perform or assist the resident with completing Activities of Daily Living (ADL). • Respond to residents call lights to provide maximum comfort, safety and privacy. • Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities). • Report any significant findings/observations to nurse immediately. • Assist residents on admission, transfer and discharge to make a smooth transition. • Responsible for the safekeeping of residents’ personal possessions. • Keep resident rooms neat and safe. • Follow proper infection control techniques • Respect the rights of each resident, particularly privacy and confidentiality. • Re-teach resident to perform ADL skills. • Provide restorative care to prevent pressure sores, contractures, and complications. • Participate in Continuous Quality Improvement and ensures follow-up occurs. • Converse with resident and family; assist them on adjustment to the community. • Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly. • Perform blood pressure, pulse, respirations, intake/output and weights accurately. Qualifications: • Certified and state tested as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing.
