Maintain accurate and timely documentation of wound assessments, treatments, and outcomes. Ensure resident confidentiality is maintained at all times in accordance with HIPAA and facility policies. Collaborate with interdisciplinary teams to support healing, prevention strategies, and continuity of care. Qualifications for Lead Wound Care Nurse: Must possess a current, unencumbered, active LPN or RN license in the state of Virginia Wound Care Certification strongly preferred but not required. Valid BLS Certification (CPR) required. Minimum of 2 years of prior experience as a Wound Care Nurse in a nursing home or long-term care setting required. Strong clinical assessment and critical thinking skills. Ability to work effectively in a fast-paced healthcare environment.
