The four providers to join the Pod will work together to co-manage a panel of patients, supported by their supervising physician and a different preceptor for each session. Description Countryside Health Care of Milford currently has LPN positions available on the 3-11 & 11-7 shifts. We are looking for dynamic, team oriented LPN's who enjoy working with a variety of patients to assist them with obtaining optimum function either within the LTC setting or to assist them with their successful return to home from our active Rehabilitation Unit.
