Assists RN in enema procedures. Collects specimens as directed by RN. Informs RN of any audible clinical alarms such as IV pumps, feeding pumps, ventilators, bedside monitors Assists with moving, lifting, positioning and transporting of patients. Assists with removal and security of patient personal items prior to procedures, as needed. Changes linen in accordance with policy and as necessary. Takes and electronically records blood pressure, temperature, pulse, pulse oximetry, and respiration per care level standard and as directed by the RN. Reports abnormalities to RN. Weighs patients and electronically records on admission, as ordered, and/or as directed by RN. Measures and records oral intake, urinary output, bowel elimination, gastric and other drainage onto I&O record (form or electronic). Calculates 8 hour totals. Empties drainage bags. Performs post mortem care. Performs safety observations for patients on continuous observation or in restraints, per policy and as directed. Reports any changes observed in the patient?s condition to the RN. Responds to and meets patient/family needs and customer concerns as soon as possible with compassion and understanding. Established effective, caring relationships with staff, patients, family and other members of the health care team. Maintains competence through in-service and educational programs. Attends staff meetings and reviews/signs minutes of meetings not attended. Maintains a safe and uncluttered environment of care in patient rooms including removing excessive furniture, supplies and equipment. Assists with supply and equipment management of patient, procedure and supply rooms per department standards. Maintains cleanliness of utility rooms and pantries as assigned. Performs other duties such as errands, relieving other units, bringing stretchers and assisting other members of the health team, as directed. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as prescribed Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals’ responses to medication or changes in health status. Serves as the nursing representative on a treatment team; provides information on nursing needs; and consults and coordinates with treatment teams and other health care disciplines to assess, plan, implement, or evaluate individuals’ care. Makes clinical rounds to assess individuals’ nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs. Implements infection control protocols and procedures and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities.
