Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Care Coordination Coordinate referrals to specialists, medical oncology, radiation oncology, genetics, nutrition, social work, and other support services. Schedule diagnostic testing and ensure timely completion. Ensure continuity of care across outpatient, inpatient, and procedural settings. Clinical Documentation Document nursing assessments, interventions, patient education, and communications accurately in the electronic medical record (EMR). Maintain accurate medication reconciliation and allergy documentation. Process medication refills according to practice protocols. Complete disability forms, FMLA paperwork, and other clinical documentation as appropriate. Telephone Triage Assess patient concerns using evidence-based triage guidelines. Provide clinical advice within scope of practice. Escalate urgent or emergent concerns to the provider. Document all patient communications in the EMR. Quality and Compliance Maintain compliance with HIPAA, OSHA, CMS, and organizational policies. Participate in quality improvement initiatives. Assist with infection prevention and patient safety activities. Maintain current nursing licensure and required certifications.
