Triages calls related to patient care, including physician orders, staff issues and other clinical issues. Job Description Minimum Qualifications: 1. Licensed Practical Nurse (LPN). 2. Three (3) years of community health experience required or combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience required. Preferred Qualifications: 1. Prior experience in telephone triage preferred. Demonstrates expertise and knowledge in a variety of areas, including, but not limited to: Current methods of care and treatment; Policies, procedures and protocols which are specific to the Agency; The Agency Infection Control Plan; A working knowledge of all reimbursement criteria; A working knowledge of all regulations; Has a working knowledge of the Medicare Conditions of Participation; and Assisting with individual caseload reviews or other audits as assigned by the manager or Clinical Director. 7. Maintains a calm and professional demeanor and customer service-oriented approach during all interactions with patients, patient family members, and colleagues. 10. Works collaboratively with all disciplines, facilitating productivity, cooperation and efficient operations. 11. Assists in clinical manager’s and scheduling coordinator in appropriate coverage based on census, skill mix, geographic location, patient need and clinical capacity. 12. Triages calls from physician and follows through on required actions, including communication to field staff, changes in orders, scheduling prn visits, and f/u as clinical resource to clinical staff if needed. 13. Utilizing voice mail and e-mail, communicates with others, as necessary in an effective and efficient manner. Responds to inquiries in a timely manner. Documents appropriately. 14. Consults with Managers regularly, providing information about progress and concerns. 15. Provides documentation which is concise, accurate and informative, and which supports reimbursement. 16. Assists with reviews of clinical reports such as timelines of care and other audits as assigned by the Clinical Director. 17. Represents the Agency at community and/or professional organizations. 18. Practices in accordance with standards of care, policies, procedures, and scope of practice, demonstrating an awareness of regulatory guidelines. 19. Demonstrates a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise. 20. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Tufts Medicine Care at Home policies and procedures. 21. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines. 22. Participates in all mandatory in-services. Physical Requirements: 1. As a Hospice RN Case Manager, you will play a vital role in delivering patient-centered care that prioritizes comfort, dignity, and quality of life. In this autonomous yet highly collaborative role, you will lead care coordination across an interdisciplinary team, build meaningful relationships with patients and families, and ensure each individual receives thoughtful, personalized support during one of life’s most important transitions. Utilizing the nursing process, the Home Health Registered Nurse will manage and deliver comprehensive home health services, including assessments, interventions, and supportive care to clients within their place of residence. Depending on the circumstances, duties may also include telephone triage, problem solving, patient/caregiver advocacy and support, with emphasis of avoiding hospitalization. As a key member of the health care team, this position must interact courteously and effectively with patients and their families as well as with coworkers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care while maintaining their own safety in the home and the community at large. Location: Lowell, MA Hours: Full-Time, Days, 40 Hours Minimum Qualifications: 1. Massachusetts RN Licensure. 2. Valid state issued Driver’s License. 3. One (1) year of acute medical/surgical nursing experience. 4. Cardiopulmonary Resuscitation (CPR) Certification. Adapts to new and unusual situations without affecting work performance negatively. Utilizes Security when and if needed for any potential unsafe situations. Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge. Completes physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies. Reconciles medications with patient and physician consistently. Demonstrates ability to cope with patient/family emotional stress and provide appropriate supportive care. Effectively manages assigned caseload, within the team model of care delivery. Establishes a daily work plan based upon patient/family priorities of service and total area needs. Promptly triages patient visits, messages, and phone calls according to priority and urgency. Demonstrates excellent physical assessment and care planning skills. Demonstrates current knowledge of pharmacology and medication administration and reconciliation. Demonstrates ability to cope with patient/family emotional stress and provide appropriate supportive care. Effectively manages assigned caseload, within the team model of care delivery. Establishes a daily work plan based upon patient/family priorities of service and total area needs. Promptly triages patient visits, messages, and phone calls according to priority and urgency. Coordinates care and discharge planning with other team members during case conferences. Acquires and maintains an expert working knowledge of all third part payers and regulatory bodies and agency policies on issues related to documentation and care provided. Communicates and collaborates with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions needed. Schedule: Per Diem Pay Range: $36.78 - $73.57 / Hourly Shift: Day shift Hours: 7:30-5p About This Role Job Summary: Reports directly to unit manager but works collaboratively with the department of anesthesia, this role is expected to gather appropriate patient information, including but not limited to: tests, labs, consults, etc. They are to ensure these results are within acceptable range parameters and, if not, collaborate with the appropriate physicians to rectify the areas of concern so surgery is not cancelled. In conjunction with support staff, they are responsible to assemble the patient chart for the surgical procedure, ensuring completion of all ordered test, and to instruct the patient on fasting times, medications to be taken/held, and answer any questions the patient may have about the procedure/process. The attire will be business casual with a lab coat.
