Job Overview This position focuses on an area of a specific medical specialty or chronic care condition (i.e. endoscopy, enterostomal therapy, mental health, total parenteral nutrition, bone marrow transplant). Serves as a resource and educator to patient, medical and hospital staff within the specialty. Coordinates care efforts for a safe, effective, efficient and patient centered transition along the health continuum. Provides pro-active disease management using evidence based practices. Acts as a liaison between patient/family, medical staff and others involved in treatment, promoting patient centered care based on jointly determined health care goals and priorities. Responsibilities Accesses, analyzes, plans and provides follow-up care. Facilitates timely referrals and fosters positive relations with community providers. Provides education, counseling, medication management, and general health information to patients and patient representatives. Triages patient concerns and responds to patient concerns between visits under provider direction, to reduce relapse or inpatient care. Functions as a resource to patient care units, outpatient clinics and medical staff regarding the clinical specialty. Identifies need and assists in writing and implementing policies and procedures. Provides education services to patients and their families and other hospital staff regarding the clinical specialty. Develops content, resource materials and coordinates the educational plan. Serves on task forces and committees related to the clinical specialty. Keeps up to date on technological advances and new pharmaceutical products. Consults with other areas regarding specialty. Assists with research projects which may include collection data, data analysis and evaluation. May identify research projects. Manages database to track patient outcomes and other specific categories as directed by physician. Promotes program and assists with marketing efforts. Oversees or manages prior authorization process and results verification process to ensure positive image. Provide effective educational material. Serves as a community liaison. Job Overview Fairview is hiring a Registered Nurse (RN) for our ICU - Cardiovascular Department at our M Health Fairview University of Minnesota Medical Center - East Bank location in Minneapolis, MN. We care for patients who have had coronary artery bypass, valve replacement and transcatheter aortic valve replacement, aortic arch repair and heart and lung transplant. The CVICU also handles patients with decompensated heart failure, adult congenital heart defects, cardiogenic shock, advanced pulmonary hypertension and cardiac arrest. We apply extracorporeal membrane oxygenation (ECMO), continuous renal replacement therapy (CRRT), ventricular assist devices (VAD), intraortic balloon pumps (IABP) and Swan-Ganz catheters routinely. Our complex patient population requires hemodynamic monitoring, critical medication management, management of organ failure, maintenance of systemic perfusion and high technical skills. Job Overview Fairview is looking for an upbeat and energetic Registered Nurse for our Emergency Department at Fairview Ridges Hospital located in Burnsville, MN. The hospital is just 20 minutes south of Minneapolis, MN near the 35E and 35 W conjunctions! It has been providing state-of-the art care and community services to the residents of the scenic Minnesota Valley since 1984. Our ED is a Level 3 Trauma Center. We have 45 beds and a yearly census of approximately 67,000 and growing! Team members include Board Certified Emergency Physicians, Advanced Practice Providers, Registered Nurses, ER Techs, Health Unit Coordinators, transport, mental health associates and Case Managers. Emergency Department Registered Nurses provide emergency nursing care for a diversified population of all age distributions (infant to geriatrics) and all problem types in a very lively Emergency Room. The Fairview Ridges ER processes 72,000 cases per year, with an average of 203 patients seen daily. The position duties include assessment, care planning, interventions, and instructing. New positions added due to volumes/growth. The patient population served by the Emergency Department consists of newborn, pediatric, adolescent, adult, and geriatric patients requiring or seeking medical care. Twenty percent of our ED volume is pediatric. Schedule: This position is 60 hours per pay period, 12 hour night shifts (7pm-7:30am) with every third weekend (Friday, Saturday and Sunday). The Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act.
