In this role you will be responsible for the safe administration of all prescribed medications and treatments as listed on the Medication Administration Record (MAR) and Treatment Administration Record (TAR), provides personal are and general nursing care to residents as needed or assigned, ensures delivery of care and services to all residents in the community to include Activities of Daily Living (ADLs), room cleanliness, scheduled activities and dining services and individual service plan need of each assigned resident.Job DescriptionResponsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director.Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies.Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days.Notify supervisor when supply is less than 2 days and medicine hasn’t been received.Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies.Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications.Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used.Responsible for weekly inspections of all medications to ensure all expired medications are removed.Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies.Responsible for removing all discontinued medication from med cart and giving it to the supervisor.Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc.Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights.Document appropriate information in the daily log as required and notify supervisor of all concerns.Answer calls and assist residents with needs as assigned.Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts.Attend and participate in staff training and in-services.Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines.Hourly Pay Rate: $24.00.QualificationsLevel of Formal Education: A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience Years of Experience: 1+ months of experience Type of Experience: Senior Living preferred Special Certifications: Current Medication Technician certification in NJ. The responsibilities of this job include, but are not limited to, the following: Locating patients, obtaining accurate vital signs by visual and auditory measures, triaging patients, assessing patients’ medical condition and preparing patients for physicals and medical treatment by Physicians and Nurses; Electronically ordering lab, x-ray, and medical supplies as directed; Notifying the Nurse and/or Physician when patient is ready for exam while communicating symptoms and relaying questions about lab or x-ray needs; Assisting Nurses and Physicians with various forms of patient treatment and care; Handling Physician and Nurse requests with respect and in a timely manner; Sterilizing and setting up instruments; Washing, drying, folding, and restocking laundry. Overview Kelemen 10 is a medical-surgical unit that cares for patients who have various needs and disease processes. Both North and South 10 have Telemetry capabilities for our patients. Each side houses 36 patients to a maximum of 72 patients. South 10 has a 12-bed Intermediate Care Unit embedded within the 36-bed unit. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. Education Requirements Graduate of NLN accredited School of Nursing. First Pre-Admission Testing, located in 3 Cooper calls to review a patient’s health information prior to a surgical procedure. Once approval for the recommended surgical procedure is finalized and patient information is reviewed the patient is provided with information on when and where to report. Once arrived and registered the patient is escorted to the appropriate location to be prepped for surgical procedure. These RN’s prep the patient for the procedure. They review medical history, medical procedures and place an IV. Patients are met by Anesthesia and their Surgeon. Once everything is reviewed and verified the patient then goes to the OR. Major Duties and Responsibilities Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follows the nursing model to promote holistic care for the residents. Ensures that policies and procedures are complied with by nursing personnel assigned. Participates in the admission, discharge and transfers of residents as required. Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. Transcribes physician orders to medical record and carries out orders as written. Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated. Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated. Schedules follow up appointments for residents and transportation needs as indicated. Provides nursing leadership to nursing personnel assigned to the unit. Provides direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures, as assigned. Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties. Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met. Participates in management team meetings as indicated to discuss census changes, resident changes in status or care need, or complaints or concerns. May assist in evaluating work performance of nursing personnel and implements discipline according to operational policies and as directed. Informs appropriate departments of concerns or complaints and files grievances as necessary. Reports any incidents or unusual occurrences to the unit manager, assistant director or nursing or director of nursing and participates in the investigative process as needed. Participates in end of life care as required. Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs. Initiates, reviews and updates care plans as required. Promotes teamwork, mutual respect, and effective communication. Participates in the survey process when inspection surveys arise. May participate in QAPI activities as needed or assigned. Ensures that infection control principles are followed and helps correct deficiencies in practice as noted. Other duties as assigned Experience and Education :Required Qualifications Prior experience in long-term care preferred but not required. A Nursing degree from an accredited college or university. Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state. Current CPR certification
