Job Summary: Summary Renders nursing care to patients within an assigned unit of the hospital, in support of medical care as directed by medical staff, RNs and LPNs, and pursuant to objectives and policies of the hospital. Essential Duties and Responsibilities Supports and promotes the mission and values of Covenant Health Ministry. Practices according to the ANA guidelines of care as adopted by the Nursing Division. Adheres to the Nurse Practice Act of NH. Performs various patient care activities and related services necessary in caring for the personal needs and comfort of patients. Documents all care provided to the patient in the Electronic Health Record. Such as Vital Signs, Intake, Output, Mobility, pain, and skin abnormalities. As well as important information related to interactions with the patient and/or visitors. Reports any unusual or changes in patient condition to the primary RN and/or charge RN Completes bedside shift hand-off report routinely and independently. RN has working understanding of guidelines provided in physician orders for safe sheath removal when patient has been given blood thinners prior to sheath removal. Demonstrates the ability to identify criteria for equipment including Intra-Aortic Balloon Pump (IABP), IMPELLA etc. Assist MD with set up, insertion and maintenance of equipment as needed. Assist with insertion of Flow wire, Opsens and IVUS. RN must demonstrate contraindications and reversal for IV adenosine infusion. Demonstrates a working knowledge of hemodynamic waveforms. Obtains the ability to monitor arterial waveform during left heart catheterization. Able to identify damping and ventriculization of arterial waveforms. Able to correctly identify different hemodynamic waveforms during right heart catheterizations. Works as a team member to support /collaborate with Care Coordination Team for monitoring level of care, and for identification of trends in clinical practices and opportunities for improvement. 4. Works with patients and families to establish objectives, goals and an action plan to meet the patient’s individual needs within the indicated level of care. Collaborates with the physicians, other care providers and community resources in the care planning process. 5. Contacts third party payer to clarify benefit eligibility, coverage, and available resources for patients. May negotiate conversion of benefits for previously non-covered services. Researches alternate sources of funding for non reimbursed services that are part of the patient’s treatment plan. 6. Coordinates, implements, monitors and evaluates outcomes of the discharge plan. Evaluates progress of this plan with the discharge planner, patient, family and the patient’s health care insurer. Goals, interventions and time frames are revised as needed as a means of assisting the patient to achieve his/her optimal level of health and functionality 7. Continually evaluates the patient’s level of care. Educates the patient, family and care team regarding resource options, facilitates decision making, initiates plans, and secures resources for the patient on an ongoing basis in complex cases. 8. Participates in quality improvement on a system level through identification of opportunities for improvement, collection and analysis of data, and participation in the development and application of revised processes. Supports departmental and medical center-wide performance improvement programs such as implementation and monitoring of clinical practice guidelines. 9.
