As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You'll also develop the patient's Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Primary Responsibilities Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days Initiates and coordinates the plan of care Documents problems, appropriate goals, interventions, and patient/family response to hospice care Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily Instructs and supervises the patient/family in self-care techniques when appropriate Maintains accurate and relevant clinical notes regarding the patient's condition You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) on the Surgical Intensive Care Unit. The Surgical Intensive Care Unit (SICU) Advanced Practice Provider (APP) is responsible for the comprehensive management of critically ill adult patients in collaboration with attending physicians, fellows, residents, nursing staff, and multidisciplinary team members. The APP provides evidence-based critical care to patients with complex surgical, trauma, transplant, and emergency general surgery conditions. In this role, you will provide direct care to patients, in collaboration with the multidisciplinary health team in accordance with privileges approved by the credentials committee, to include identification, management, and/or referral of health problems. Conduct comprehensive or episodic health assessments and develop patient care plans within clinical practice and medical care protocols. Take patient histories, conduct physical examinations, and record the date in the patient's record; screen patients to determine the need for hospital admission, making referrals to other disciplines as indicated, write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients.
