Job Summary The Licensed Practical Nurse – Specialty Office provides general nursing care to patients, families and caregivers in the Medical Group practices. The LPN assists and advocates for the patient and family’s health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/providers, registered professional nurses and multidisciplinary care team members. The Licensed Practical Nurse can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall be performed in accordance with the standards outlined for Licensed Practical Nurse in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. Maintain a cooperative relationship among the healthcare team by effectively communicating information. PURPOSE AND SCOPE: Ensure the provision of quality patient care to all hospitalized patients in the most efficient manner in accordance with company policy. Provides direct supervision and coordination of inpatient staff. Responsible for mid volume monthly treatments. Job Description The Department of Ambulatory Care Services at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner, Dermatology. The successful candidate will: Provide same-day procedures, infusion/laser referrals. Document all care provided in the patients' medical records. Develop further, maintain/manage patients care panel with participation in managed care programs and other direct patient care programs as applicable. On a daily basis, review all laboratory and radiology reports to identify abnormal results for urgent follow-up. Ensure that patients are contacted to arrange additional care and that their primary providers are notified. Participate and provide departmental and Family Medicine educational activities to medical students, allied health professional students. Participate in hospital and departmental health fairs, community health education events and speak to community groups, when requested. Attend staff meetings and, as required, write nurse practitioner protocols, in collaboration with the Medical Director. Stay abreast of developments in Dermatology health care.
