This position will provide comprehensive, high-quality primary, episodic and/or recuperative care at our main campus and surrounding shelter-based clinics. The ideal candidate for this role must be a self-starter and have the ability to lead a care team in partnership with other clinical peers. Our practice is collaborative, supportive and innovative. Responsibilities: Provides comprehensive, high quality episodic and/or recuperative care at various clinical sites staffed by Boston Health Care for the Homeless Program. May work at shelter-based clinic, hospital-based clinic, respite facility or on street-based outreach based on program needs. Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program. Refer patients as appropriate to medical specialists, hospital admission, recuperative care, and detox facilities, providing appropriate information for continuity of care. Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts. Provide education to patients on health promotion and disease treatment and prevention. Work as part of multi-disciplinary team (medical, nursing, dental, BH, pharmacy, case management) in caring for patients. Administrative Responsibilities: Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Maintain active DPH, DEA, and professional license, managed care credentialing and CPR training. Some on-call and holiday coverage required. Qualifications: Licensed by the Commonwealth of Massachusetts. Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency. At least one year of direct NP/PA experience. Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to start date, applicant must submit certification within 90 days of start date. Experience in working with underserved populations, mentally ill, homeless and/or persons with substance use disorders preferred. Bilingual proficiency preferred. PURPOSE AND SCOPE: Manage and oversee the daily operations of the facility, ensuring cost-effective facility operations in accordance with all legal, compliance and regulatory requirements and programs. PRINCIPAL DUTIES AND RESPONSIBILITIES: Clinic Operations: • Provides leadership, coaching, and development plans for all direct reports. • Partners with internal Human Resources, Quality, Education, and Technical Services departments. • Collaborates with the Home Therapies Program Manager to oversee the facility’s Home Therapies Program. • Maintains integrity of medical and operations records and complies with all data collections and auditing activities. • Accountable for completion of the Internal Classification of Disease (ICD) coding. • Responsible for all required network reporting and on-site state or federal surveys. • Manages the tactical execution of short- and long-term objectives through the coordination of activities with a direct responsibility for results, including costs, methods, and staffing. • Responsible for the administration of the daily business operations of the dialysis clinics including managing the functions and actions related to the center staff, quality of overall provision of patient care, maintenance of the physical plant and center equipment, and inventory control. • Manages the profit and loss and other related financial aspects for the center, ensuring optimal facility operations to achieve or exceed the budget and key performance indicators. Position Responsibilities: Ensures all care delivered is within the scope and established standards of safe professional nursing practice with core care competencies Collaborates with other members of the multidisciplinary team (MD, NP, nurses, social worker, therapists, dietician) to meet the clinical needs of the patient and monitor outcomes of care Adheres to best practice recommendations and implements evidence-based interventions to ensure optimum outcomes for patients Practices in a primary nursing model and maintains accountability for a set of assigned patients with RN oversight of assessment and care planning Collaborates and participates in care planning activities and supports autonomy and individual choice wherever possible In collaboration with the RN, ensures the plan of care is an accurate reflection of the patient’s individualized needs and ensures that the plan of care is followed and revised as necessary. Documents pertinent information regarding care delivered in designated areas of patient’s record using the electronic medical records. Initiate necessary reports including DPH, incident and concern resolution reports Identifies and seeks out necessary resources as needed to ensure the provision of safe and effective care Delegates and supervises clinical care delivered by the nursing assistant Assumes accountability for his/her own learning needs and professional development Demonstrates critical thinking abilities related to problem resolution Role models professional behavior Actively participates in shared governance, committees, etc Collaborates in the development of new work processes and systems Serves as a resource and support to the PCAs in the unit Identifies and responds to safety concerns on the unit Maintains compliance with policies, procedures, practices, regulatory matters and HIPAA compliance. Participates in the collection and monitoring of quality data to ensure high standards of care Participates in performance improvement projects Participate and assists with unit activities as needed (i.e. assist with serving meals) Serves as a coach and mentor to patient care associates May be assigned charge nurse responsibilities with RN oversight. Responsibilities may include: Giving shift report to on coming staff Provide clinical/supervisory oversight to the unit during a shift Delegate assignments to team assigned to shift Audit/assure completion of admission or discharges during shift with the RN’s cos-sign Attend rounds (wound, physician etc) If acting in the capacity of charge nurse, may develop patient care assignments (appointing other staff, RN, LPN or PCA to implement an established care plan for a designated period of work time), create schedules, and contribute to performance evaluations and educating other staff on HSL policy and procedure. Qualifications Graduate of approved school of nursing required Current Massachusetts license as an LPN in good standing Previous experience preferred Understand the philosophy and support the model of care at HRC required Must be professional, proactive, collaborative, conscientious and results-oriented individual. Must have an optimistic and positive demeanor, excellent oral and written communication skills, good intuition and able to adapt to changing priorities and display good, sound judgment Superb organizational skills. Must have solid analytical skills. Must have ability to innovate, think strategically and conceptually, manage multiple projects simultaneously and handle even difficult situations.
