The Nurse Care Manager 1 contributes to the team's effectiveness by reviewing plans of care, identifying barriers to effective and efficient utilization of resources, and appraising patients' psychosocial, financial and clinical needs throughout the continuum of care. The Nurse Care Manager 1 functions as a liaison between patients, physician practices, the hospital, and the community and ensures that patients' continuity of care needs are met. Provides basic assessment of patient's psychosocial, financial, cultural, and family situation. Coordinates social services for patients and families to address needs. Collaborates with inpatient staff and physician groups to determine goals for length of stay management and discharge management activities. Promotes cost-effective quality care services Monitors and reports potential quality issues and makes recommendations when appropriate. Utilizes the CM information system and appropriate technologies. Monitors variances and trends related to select goals. Assist CM team and hospital groups to develop action plans and address identified opportunities for improvement. Promotes customer satisfaction among patients, families, physicians, external case managers, payers, vendors, and inpatient staff. Actively promotes ICARE principles. Complete work assignments with C.N.A.'s, and communicate any changes in resident care plan or condition of resident. Maintain PHI by assuring C.N.A.'s adhere to facility policy and procedure. Regulatory Compliance: Understand, comply with and promote all rules regarding resident rights. Educate and assure C.N.A. compliance with respect to resident rights. Report all suspected, alleged, or witnessed physical, sexual, or verbal abuse immediately to the Administrator, Abuse Prevention Coordinator and/or designee. Training and Development: Attend all staff meetings and in-services as required. Attend organization-wide programs and meetings to be informed and current on organizational strategies and programs. Certified Nursing Assistant (CNA) Duties and Responsibilities: Under the direction of the Director of Nursing or CNA Supervisor, the CNAs work closely with the nursing staff to ensure coordination of patient care. Provides physical support for patients/resident’s daily activities and personal hygiene, including bathing, dressing, grooming, transferring to/from bed, toileting, walking, standing, or exercising. Turns and repositions bedridden patients. Ensures proper resident/patient diet based on their dietary restrictions. Measures and records food and liquid intake. Examines patients for open wounds, bruises, or blood in urine. Notes and communicates patient/resident behavior and changes in mood. Gathers medical supplies. Cleans rooms and bed linens. Checks and records vital signs, including blood pressure, pulse, temperature, and respiration rate under the supervision of a licensed nurse. Answers to patient calls and needs. May aide patients being transported to and from appointments. All other duties, as assigned. Certified Nursing Assistant (CNA) Skills and Abilities: Good collaboration and teamwork skills with the nursing staff. Familiarity with symptoms and signs of improvement for a number of medical conditions. Ability to read, write, speak and understand English. Meets all health requirements, as required by law. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications. Certified Nursing Assistant (CNA) Education and Experience: High School Diploma or GED. State Certified Nursing Assistant License/Certification. Current CPR certificate or become certified within 90 days of employment, preferred. Some facilities may require this. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
