Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Contributes to professional development of self. · Maintaining current professional status and licensure. · Adhering to professional standards of conduct and clinical code of ethics and practice. · Implementing in-services and training. · Maintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers. · Meeting continuing education requirements of regulatory agencies. 5. Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Examples Of Typical Tasks Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. Participates in defining, implementing, maintaining and interpreting standards of nursing practice. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. Participates in determining conditions, resources and policies essential to the delivery of nursing care services. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Performs other professional nursing activities as delegated by nursing service administration. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
