Job Description Under the guidance of the Patient Capacity Management Center (PCMC) Leadership, to include Central Patient Placement and Transfer Center (CPP/TC) RN leads and supervises a team, ensuring efficient and safe patient placement and flow within the UCSF Medical Center Health System. This role involves collaboration with various healthcare professionals and departments to manage patient admissions, transfers, and discharges, while maintaining a positive patient experience and high-quality care standards. Responsibilities Actively manages inpatient unit staffing levels; ensuring staff coverage is appropriate to each departments’ current needs. Assumes responsibility for the unit in areas delegated and in the absence of the Unit Director Assists with the management of patient care administered by other members of the nursing team and acts as a resource to assist nurses in adhering to established nursing procedures Performs other duties as assigned/required. Job Description Come join our dynamic, highly skilled Cardiothoracic (CT) Surgery inpatient APP team. The Cardiothoracic Surgery Advanced Health Practitioner position has clinical, procedural, administrative, and educational responsibilities in the care of adult patients requiring cardiac and thoracic surgery. The CT surgery service provides advanced surgical treatments, including coronary artery bypass grafting, aortic and mitral valve replacement, extracorporeal membrane oxygenation, and heart transplantation. We value individuals who are professional, with strong interpersonal and communication skills, and able to prioritize and adapt to changing situations. Our CT surgery APPs manage patients collaboratively with CT Surgery Fellows and Physicians, perform bedside procedures, and assist in the operating room. The CT Surgery APP assists in the development of practice protocols, evaluation of quality of care, and education of patients, families, and other health care practitioners. Responsibilities 90% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1.Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. FUNCTION Cares for residents under the direction and supervision of a registered nurse or a licensed practical/vocational nurse. MATHEMATICAL SKILLS Ability to add and subtract three digit numbers; calculate percentage of whole; measure in cubic centimeters (cc’s); measure in feet and inches. Ability to perform these operations using units of weight measurement, volume, length and height. REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Dressing and grooming, eating and swallowing - Maintains range of motion; proper turning and positioning in bed and chair - Provides bladder and bowel retraining - Instructs the resident on the care and use of prosthetic and orthotic devices - Incorporates training and skill practice activities which include repetition, physical or verbal cueing and task segmentation • Care for residents with dementia: - Knows how to respond to residents’ behaviors - Allows residents to make personal choices - Provides and reinforces other behavior consistent with residents’ dignity - Uses the residents’ families as a source of emotional support - Demonstrates a basic understanding of behavior and functional symptoms associated with dementia by providing interventions and treatments to help reduce these symptoms - Uses modification in the presentation of activities of daily living to maintain and maximize resident competencies and function - Uses effective intervention techniques to address behavioral concerns - Acts as a facilitator for activity programming - Demonstrates effective communication techniques for persons with dementia • Provides non-resident care including, but not limited to: - Washing wheelchairs - Cleaning soiled utility areas - Cleaning nurses’ station - Cleaning shower and whirlpool rooms - Cleaning kitchen areas on the nursing unit • Provides transporation of residents to and from hospital and medical appointments RESIDENT RIGHTS Promotes and protects resident’s rights, assists residents to make informed decision, treats residents with dignity and respect, protects residents’ personal belongings, reports suspected abused or neglect, avoids the need for physical restraints in accordance with current professional standards and, supports independent expression, choice and decision-making consistent with applicable law and regulation.
