Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care. Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition. Participate in patient care and make nursing visits as needed to support the clinical team. Assist with reviewing clinical documentation for completeness, accuracy, and compliance. Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation. Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families. Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team. Summary: In this role, you will float to our different health centers in Manhattan (Midtown to Downton financial center areas) performing front and back office duties. School LPN Nurse Needed in the Bronx 2 days per week, 9am-5pm! Scope of Services Consultant will coordinate and oversee the health care services across the 2 sites. This includes: -Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. This also includes managing all health and nutrition components of the programs in accordance with federal and state regulations, implementing the medication policy including administration of medication, measuring blood pressure, blood sugar and monitor records to ensure compliance with regulatory standards, and function as part of a cohesive team that develops and monitors program operations. Conducting Physical Assessments: -Assist all two center base sites with health related information for each child and review all medical information and charts. -Document visits at all sites for data collection in accordance with all guidelines for health- related child outcomes. -Review child physicals upon enrollment and on a regular basis thereafter to ensure appropriate updates and sustaining of ongoing physical updates. -Ensure all physicals have been completed within the last year. -Ensure the child is up to date with vaccinations. -Provide guidance for the completion of Child Health Care Plans and monitor every six months for update compliance, renewals, or changes in medication and to update doctor/parent permission to continue administering. -Review of any medications that are related to allergies or specific medical care instructions and note this information in the children's file.
