Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. The APP will be responsible for coordinating the care of patients in the hospital setting Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. Job Description The Heart Transplant Nurse Coordinator for the Advanced Heart Failure Comprehensive Care Center provides direct care to patients with multiple, often complicated acute and chronic medical problems. The Nurse Coordinator is the primary point person for the program in the post heart transplant phase of care and works closely with the multidisciplinary team and multiple UCSF departments. The primary responsibilities (as assigned) of the Nurse Coordinator will be to serve as the primary coordinator for post-transplant services and facilitate and assist with all aspects of patient care following heart transplantation.
