In-jail services include medical, nursing, mental health, substance use treatment, social work, dental and vision care, discharge planning, and reentry support. In addition to providing direct patient care in the jails, CHS leverages the resources of the nation’s largest municipal health care system to help discharged patients successfully return to their communities. CHS is also a pivotal partner in New York City’s criminal justice reform efforts. Duties & Responsibilities Work in collaboration with physicians to provide patients with care, education, and support. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management. Coordinating and assigning duties to other health care personnel, such as licensed practical nurses, nursing aides, and patient care associates. Responsibilities include: (Detailed Task) Comply with procedures, regulations, and rules in maintaining medical records Monitor, record and report symptoms and changes in patients' conditions. Record patients' medical information and vital signs. Modify patient treatment plans as indicated by patients' responses and conditions. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans. Monitor all aspects of patient care, including diet and physical activity. Direct and supervise less skilled nursing/health care personnel, or supervise a particular unit on one shift. Prepare patients for, and assist with, examinations and treatments. Observe nurses and patients to ensure that proper nursing care is provided. Instruct individuals, and other groups on topics such as health education, disease prevention and childbirth, and develop health improvement programs. Prepare rooms, sterile instruments, equipment and supplies, and ensure that stock of supplies is maintained. Administer local, inhalation, intravenous, and other anesthetics. Provide health care, first aid, immunizations and assistance in convalescence and rehabilitation. Conduct specified laboratory tests. Direct and coordinate infection control services, advising and consulting with specified personnel about necessary precautions. Perform administrative and managerial functions, such as taking responsibility for a unit's staff, and planning. Provide or arrange for training/instruction of auxiliary personnel. Refer patients to specialized health resources or community agencies furnishing assistance. Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing. Work with individuals, groups, and families to plan and implement programs designed to improve the overall health of communities. Triage nurse works to assess patient needs and provide clear patient information and education providing the patient with clinical advice on the management of their care Engage in research activities related to nursing. EMERGENCY RESPONSE Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, and burns as well as recovering post-operative patients Evaluate a patient’s condition in order to determine the right course of treatment When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc. Monitor patient’s condition and keep it stable while in route to the clinic Aid in the preparation of the transfer of patients by Urgicare and/or EMS to the emergency units or hospitals Document medical treatment given to the patient Keep the equipment clean after use; check and replace damaged and used supplies. Collaborate with pharmacy to replenish supplies. WORKING CONDITIONS Because patents require round the clock care, working hours include days, nights weekends and holidays. RNS are required to stand for long periods of time. Employees are encouraged to safely move patents and use alternative methods whenever possible; alternative methods include requesting assistance to roll/lift/transfer patients. RNs may face hazards from exposure to chemical and infectious diseases. In addition, they have to treat patients that are confused, irrational, agitated and/or uncooperative PHYSICAL DEMANDS Ability to lift fifty (50) pounds. Moving, lifting or transferring of patients may involve lifting of up to 100 pounds Ability to stand for extended periods Fine motor skills Visual acuity Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Department Preferences In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement: Education An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below: Bachelors in allied health discipline = $900 Masters in allied health discipline = $1,200 Bachelors of Science in Nursing (BSN) = $1,800 Masters of Science in Nursing (MSN) = $2,000 Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500 An Annual Certification Differential If You Currently Hold Appropriate Certifications That Are Nationally Recognized In The Discipline Or Area Of Specialization For Which You Are Employed, As Listed Below Certifications in the following disciplines/areas of specialization are eligible for premium pay at $3,000: Behavioral Health Emergency Surgical Services Critical Care Certifications in the following disciplines/areas of specialization are eligible for premium pay at $2,000: Med-Surgical/Dialysis Correctional Health Professional – if assigned to Correctional Health All other nationally recognized certifications are eligible for regular pay at $1,878. An annual certification differential ranging from $1,878 up to $4,878 if you currently hold appropriate certifications that are nationally recognized in the discipline or area of specialization for which you are employed. An annual nursing experience differential ranging from $987 up to $8,190 for up to 10 years of experience if you have outside nursing experience as a Staff Nurse.
