Responsibilities In collaboration with the physician and your preceptor, you will ensure implementation of the assessment plan, evaluation, and follow-up of the plan of care for the patient population. Initiating an ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the patients/families. Interpreting assessment data and information to develop a nursing care plan which contains problem identification, nursing diagnosis, nursing orders, approaches, interventions, short/long-term goals, and discharge planning. Participating as a productive member of the interdisciplinary team in assessment, planning, implementation, and evaluation of rehabilitation goals. Assuring that documents in the medical record are complete, factual, accurate, and timely. JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Safety of Residents: Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied. Restraints are checked at the beginning and end of each shift. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently.
