Resposibilties The CNA maintains a "homelike environment for the residents, protects and promotes Resident Rights and assists the person to maintain independence and control to the greatest extent possible. Provides direct care to residents to improve or maintain the resident’s abilities Follows all facility policies and procedures including those related to infection control. Records accurate, legible information about resident care and condition in appropriate sections of residents’ records. Communicates with Unit Charge Nurse about assigned residents regularly throughout shift. Monitors physical environment for safety or fire hazards and corrects or reports potential problems

This manager will manage RN case managers, CHWs, and BHAs, in accordance with established Compass (formerly Care Model) and ICP Programs, as well as all UHC requirements. Manage productivity and expected outcomes across the teams, engage staff. Must be a critical thinker and have experience in complex care management, chronic conditions, and behavioral health management to manage, lead and advise the teams as a subject matter expect. This role also requires submission of state reports, presentations and public speaking. Data trending, extracting, capturing and analysis are also an essential element of this role. Location : 8380 Old York Rd. Elkins Park, PA 19027 Hours : M-F, 40 hrs a week Specialty : Outpatient Clinic, Family Medicine Duties Treats and evaluates patients; administers advanced nursing care in a particular clinical specialty area under the direction of the licensed physician. Performs a wide variety of patient care activities including diagnosis and the prescription of medical therapeutics or corrective measures in collaboration with and under the direction of the licensed physician.
