$105,000 - $140,000 / year
Purpose: We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and complex care patients in the Philadelphia, PA region on a full-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases. What You'll Do: Provide high quality care to primary and palliative patients in and around Philadelphia, PA. Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care. Effectively communicate with office staff and clinical operations across various internal platforms Provide high quality, patient centered care to a panel of patients Develop impressions, diagnosis, and treatment plans Foster trust and consult with patients, families, and caregivers during visits Order diagnostic testing as medically necessary Prescribe medications supplies and appropriate medically necessary interventions What You'll Need: Board Certified, with a valid NP or PA certification in the state of Pennsylvania Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose Positive demeanor and strong people skills Ability to confidently problem solve and operate independently 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required) What you'll get: A flexible schedule - so you can see your patients during a schedule that works for you Autonomy - so you can feel trusted to provide the best care possible. The Medical Assistant provides basic patient care and performs job specific tasks under the direct supervision of a physician or other licensed medical provider according to established policies and procedures. Performs activities necessary to provide for the personal needs, comfort and safety of patients and their families. Responsibilities include clinical, environmental, and administrative duties in designated areas. Education High School Diploma or Equivalent Required Experience Other Graduate of Medical Assistant program. Required 1 Year Experience As An Outpatient/Ambulatory Medical Assistant Preferred General Experience with EPIC EMR Preferred Licenses Cert Clin Medical Assistant Required or Certified Medical Assistant Required or Registered Medical Assistant Required Basic Life Support Required

Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care. Works collaboratively with physician, hospital and community resources to support the patient's clinical plan of care. Education Bachelor's Degree in Nursing Required Master's Degree in Nursing Preferred Experience 3 years experience in disease or case management services with focus on telephonic management, medical reconciliation and ambulatory care coordination Required General Experience with clinical, hospital-based information systems Required Licenses PA Registered Nurse License Required Multi State Compact RN License Required
