Position Summary To fulfill the needs of patients in a variety of age groups, their family members, and the community by performing varied patient care tasks and functions in accordance with licensure, certification, training, and regulatory requirements. Licensed Practical Nurse LPN (P/T 16 hours) Great pay, Weekly Salary. Position Summary: Reporting to the Registered Nurse will provide for medication dispensing and nursing assistance in the diagnosis, and continuous primary medical care to participants. Will act and function as the HIV Coordinator of services for the clinic. Assists in the performance of admission and physical examinations. Provides the physician or physician assistant with urine toxicology and other laboratory data as they become available. Attends on a punctual basis clinic meeting. Performs any and all tasks necessary to improve the quality of care, to improve the productivity of medical staff, and to improve participants satisfaction with the medical/nursing care they receive. Performs any and all tasks necessary to enhance reimbursement and reduce denials for services rendered. Performs phlebotomy, EKGs, vital signs and assists in urine collection. The schedule is predictable with no overnight shifts Qualifications: Licensed Practical Nursing state certification required: Bachelor's Degree preferred. Three (3) years of nursing experience is preferred. Excellent computer skills and working knowledge of electronic medical records required. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Engages in cluster feeding while seated. Monitors daily intake and keeps nurses informed. Records food intake on Nutrition Work Sheet. Making Beds Uses recommended amount of linen and diapers. Disposes of linen in proper manner. Properly utilizes clean and dirty linen carts. Secures call bells to bed. Beds are made in proper manner. Knowledgeable in procedure for linen precaution. Safety of Residents: Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied. Restraints are checked at the beginning and end of each shift. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently.
