JOB TITLE : RN Supervisor REPORTS TO : Director of Nursing Services TITLE OF IMMEDIATE SUBORDINATES : Charge Nurse RN/LPN/C.N.A JOB SUMMARY: The primary purpose of your job position is to supervisor the day-to-day nursing activities of the facility during your tour of duty -overseeing of the facility's operations and management including providing optimal care, communicating the facility's policies and ensuring the general order, safety and cleanliness of the facility. QUALIFICATIONS q Ability to read, write and comprehend English; ability to follow oral and written instructions. q Interest in the needs of the aged and the chronically ill with the ability to work with both. q Ability to work hours as scheduled based on the requirements of the position/assignments. q Registered Nurse licensed in the State of employment. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred. q Knowledge of nursing theory and practice including the administration of medications. q CPR certified. WORKING CONDITIONS : Works in an appropriately lighted and ventilated environment. This position may result in your being exposed to blood/body fluids. PHYSICAL REQUIREMENTS : q Must be able to move intermittently throughout the day, proper body mechanics required—bending, stooping, turning, stretching, reaching above the shoulders and occasional lifting are involved. q Must be able to cope with the mental, emotional and physical stress of the position. q Must be able to observe, listen and communicate effectively. q Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, support agencies, and outside agencies. q Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. q Must be able to transfer and pivot residents. q Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum of 50 feet. PRIMARY FUNCTIONS : q Makes decisions relevant to the operation and management of the facility during the assigned shift. q Verifies that the resident's physician and family is notified when there is a significant clinical development in the resident's condition or if the resident requires immediate medical care. q Makes decisions concerning staffing in the facility such as replacing call outs, authorizing overtime and reassigning personnel when appropriate. OTHER DUTIES: q Assumes independent responsibility for yearly written evaluations of charge nurses and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Such evaluations become part of the charge nurses' permanent records and shall be used as the basis for determining continued employment, promotional opportunities and the amount of any wage increases granted. q Assumes independent responsibilities for documenting disciplinary concerns for charge nurses on designated shifts, including reprimands, oral and written warnings, and recommending the suspension or discharge of an employee. q Directs proper handling of resident care emergencies. q Makes house rounds and consults with charge nurse concerning new admissions and acutely/seriously ill residents. q Ensures availability of supplies and that all equipment is in proper working order. q Supervises to ensure the proper completion of accident/incident documentation and notification. q Functions as Charge Nurse RN in the event that there is a call out and the nurse cannot be replaced. q Assists in on-going Quality Assurance of resident care and documentation. q Assumes other duties as assigned by ADNS. q Conducts in-services during assigned shifts according to pre-determined schedules. Records attendance. q Monitors schedule of physicians' visits and keeps Medical Director advised of any and all discrepancies. q Projects the facility's image in a positive light to residents, staff and community. q Maintains a positive attitude and rapport with family members. q Involved in medication returns EXPECTATIONS : q Presents a neat appearance with a pleasant speaking voice and personality. Overview Provides licensed practical nursing support to the Medical Care at Home (MCAH) clinical team during in-house and virtually supported preventive assessments. The LPN, Field Clinical Support is physically present with the patient and works under the direction of the Nurse Practitioner (NP), collecting and communicating objective clinical information, completing structured intake activities, supporting Care GAP closure, performing ordered procedures and testing within LPN scope and demonstrated competency, and reinforcing patient education. Position Summary: We have an exciting opportunity to join our team as a APP Manager Park Slope Family Health Centers at NYU Langone. In this role, the successful candidate Responsible and accountable for the management and quality of nursing care delivered at the site as part of the site leadership team. Fosters an effective work environment for interdisciplinary teamwork, patient satisfaction and staff satisfaction. Makes management and budgetary decisions, sets work schedules, recruits and trains nurses and other health care professionals on the care team. Serves as a role model and resource to all staff. Function also includes providing primary care for patients at least one session a week. Job Responsibilities: Demonstrates clinical knowledge and skills in area of supervision and directs staff in its application. Provides direct patient care as needed onsite clinical support/triage/care coordination to meet patient care needs or to model behavior for staff development/training Conducts daily huddles. Establishes maintains processes to promote a multidisciplinary team approach in developing, implementing and evaluating patient care and/or family education Conducts quality improvement monitoring and participates in problem identification and solution to improve key processes/systems/patient care; shares results in staff meetings. Encourages staff participation in quality improvement activities. Attends and participates in monthly Network meetings Investigates incidents and occurrences and institutes corrective action as appropriate Promotes safety and risk free environment for patients and staff Ensures staff compliance annual and interval updates, i.e. environments of care, infection control, emerging pathogens,hazard and safety surveillance. Responds to patient/family needs in timely, positive manner and ensures service excellence among all staff Responds to staff, provider and patient satisfaction needs and identifies and implements solutions in collaboration with related departments as needed Plans for staffing needs of the site(s)/modules(s) on a long and short term basis, adjusting based on workload and other staffing variables to maintain safe, efficient care in an effective manner Prepares staff schedules in a timely manner. Monitors appropriate use of benefit time. Completes time cards for timely and correct payroll. Approves overtime Monitors site/module staffing to budget, overtime/agency use and provides justifications as needed Interviews potential staff members and recommends hiring of applicants with appropriate skills and behavior in collaboration with recruitment. Establishes work performance and clinical competency of personnel on a timely basis and makes recommendations for promotions, transfers and discharges. Takes corrective action and/or disciplinary action with staff in collaboration with Human Resources. Facilitates and ensures orientation of personnel to the site/module. Plans for experience needed to perform competently. Participates in the clinical and leadership development of employees in his/her area. Establishes work performance and clinical competency of personnel on a timely basis and makes recommendations for promotions, transfers and discharges. Takes corrective action and/or disciplinary action with staff in collaboration with Human Resources. Participates in the clinical and leadership development of employees in his/her area. Maintains accurate records and evaluates effectiveness of activities. Identifies and meets learning needs in order to maintain competency and respond to new clinical and management expectations Counsels and coaches staff concerning their work performance and development; holds staff accountable for performance. Makes recommendations for staff educational needs and plans educational activities independently and in conjunction with others. Establish pleasant working environment conducive to teamwork, growth and recognition. Utilizes effective conflict resolution strategies when dealing with staff, providers, patients and family members Maintains effective ongoing and timely communication with staff and maintains records of meetings. Makes recommendations for staffing supply and capital budget needs of the module/site. Monitors budget in collaboration with Site Director / Medical Director. Participates in annual staffing/plan budget review process. Remain current with new developments in your respective specialty. Minimum Qualifications: To qualify you must have a Master Degree required. Three years of ambulatory care experience required. Previous leadership experience preferred. Willingness to devote the time required completing assigned tasks on schedule. Valid New York State Nurse Practitioner license, NYS Infection Control Certificate. Basic Life Support (BLS) Certification through the American Heart Association is required at the time of hire and is required to be maintained.. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
